What you'll be doing
As a Senior Primary Care Nurse, your responsibilities will include:
1. Leading a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions.
2. Undertaking medication rounds, dispensing controlled medication, attending emergency response situations, assessing patients, and undertaking reception screening.
3. Providing outstanding evidence-based care, seeking out, listening to, and acting on patient feedback to ensure care is personalized and informed by what matters to them.
4. Delivering non-judgemental care that makes a real difference.

Qualifications required
* Registered nurse - Adult NMC.
* Evidence of continued professional development.
* Understanding of multi-disciplinary and multi-agency working complexities.
* Excellent communication skills.
* Proactive problem-solving skills.
* Motivated, both individually and within a team.
Experience in a hospital, community, or prison environment is advantageous but not essential. Working in secure environments is challenging yet rewarding, offering opportunities to develop healthcare skills daily.

Working Hours
Monday - Sunday service, with shifts including:
* Weekdays: 7:30 am - 5:30 pm or 6:30 pm.
