About the role
You’ll empower your clients to take actions for themselves or advocate on their behalf for better service, support, and outcomes, so they can live their life with Parkinson's in the way they choose.
You’ll deliver tailored information and support through various channels including telephone, video, email, and, where appropriate, home visits and community settings.
You’ll be responsible for building excellent relationships with healthcare professionals, relevant agencies, volunteers, and groups.
What you'll do:
* Provide professional, person-centred, in-depth support to clients through various means, ensuring efficient resource use.
* Support clients on health and social care issues, including emotional, employment, and welfare benefits, advocating on their behalf.
* Respond flexibly to enquiries across different channels.
* Collaborate with clinicians, nurses, and other professionals to enhance service support and outcomes.
What you'll bring:
* Experience managing and delivering individual casework, advocating for clients, and navigating health and social care systems.
* Understanding of safeguarding policies and procedures.
* Ability to remain calm and use emotional intelligence in challenging situations.
* Proficiency with digital tools and online case management systems.
* Ability to build effective relationships within multidisciplinary teams.
Additional requirements:
* You must reside in the specified area to perform this role.

* Interviews are scheduled for the week commencing 19th May 2025.
* The successful candidate must:
o Live in the specified area (South East Northumberland, North Tyneside, South Tyneside, Sunderland).
o Be able to travel within and outside the area without reliance on public transport.
o Preferably hold a full driving license.
o Have a broadband connection with at least 2Mb download speed.
o Undergo an enhanced DBS check; refusal will result in withdrawal of the offer.
