What You Will Do:
* Provide high-quality nursing care to patients in their homes and community environments.
* Work collaboratively with multidisciplinary teams to create tailored care plans.
* Conduct health assessments and monitor patient wellbeing.
* Deliver health education and practical advice to patients and families.
* Maintain accurate and up-to-date clinical records in line with NHS standards.
