Main duties of the job
* Assess, diagnose, and manage patients with acute and chronic conditions

* Assist with Anima Total Triage
The post holder, acting within their own professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision‑making and expert care, including assessment and diagnostic skills, for patients within the Practice in the clinic setting and undertaking home visits for patients who fall within their clinical remit.
You will be supported daily by a supervising GP who you will have constant contact with as required.
Job responsibilities
Hours are negotiable and flexible, full or part‑time considered.
Salary is dependent on experience but based on Band 7‑8 agenda for change rates.
Accountable to: The GP Partners and Advanced Paramedic Practitioner.
Key responsibilities and duties include but are not exhaustive:
— Assisting GPs by seeing and triaging patients requesting same‑day appointments and then assessing, diagnosing, and developing management plans for those presenting with an undifferentiated diagnosis in a booked clinic which may involve both face‑to‑face and telephone consultations. The following exclusions apply initially: children aged < 1 year, pregnant women, skin lesions, and gynaecological/womens health problems.
— Liaising with the duty doctor to ensure prescribing of appropriate medication.
— Dealing appropriately with patients presenting at the Surgery who require immediate or emergency assessment and initiating effective emergency care.
— Initiating, requesting, and interpreting appropriate investigations and referrals.
— Undertaking some home visits within the practitioner’s competence.
— Assessing patients’ needs from a physiological and psychosocial perspective and planning clinical care accordingly.
— Assessing and developing management plans for patients with complex needs (including the housebound and those on the Frailty register).
— Supporting patients to adopt health‑promotion strategies that promote healthy lifestyles and applying principles of self‑care.
— Developing integrated working with the GPs and MDT within the Surgery and in the community and coordinating meetings as required.
— Providing hands‑on treatment‑room assistance if needed.
— Venepuncture of patients being reviewed.
Administration and Professional Responsibilities:
* Ensure accurate notes of all consultations are recorded on the clinical computer system.
* Ensure accurate completion of all necessary documentation associated with patient health care.
* Collect and maintain statistical information required for regular and ad hoc reports and audits.
* Restock and maintain tidiness of clinical areas and consulting rooms when required.
* Monitor laboratory results and act according to a defined protocol (when time permits).
* Review and triage Docman documents (when time permits).
Training and Personal Development:
Supervision and mentoring will be provided by one of the GPs or the Advanced Paramedic Practitioner for each clinical session undertaken. Learning outcomes will be discussed and recorded by the practitioner. Personal development will be encouraged and supported. Training, development needs and performance will be assessed by an annual appraisal. It is the individual’s responsibility to meet requirements for continued professional registration, remain up to date with new guidance, developments and to complete mandatory training. Attendance at in‑house clinical governance and other training meetings will be expected. Work closely with all clinical and non‑clinical members of the Practice Team to promote the smooth running of the Practice. Establish and maintain good working relationships with the wider primary health care team, community health workers and secondary care departments.

All confidential information relating to patients, their carers, practice staff and other health care workers must be regarded as strictly confidential. Information relating to the Practice as a business organisation must also be treated as confidential. Adherence to the Practice confidentiality and information governance policies is expected at all times.

The post holder has a responsibility to comply with the Health and Safety at Work Act 1974 and to identify risks involved in work activities and undertake activities in a way that manages these risks. All duties should be carried out in a manner that promotes the principles and practices of infection prevention and control in compliance with national standards and Practice policies.

Person Specification
Experience
* Post‑registration qualification to evidence autonomous practice
* Understanding of patient management in primary care setting
* Experience of working in a multidisciplinary team in an acute Primary Care setting
* Sound knowledge of local primary care services
* Administration of medications within Patient Group Directives
Qualifications
* Registered Paramedic with the Health and Care Professions Council (HCPC)
* Degree in Paramedic Science (BSc or equivalent)
* Evidence of continued professional development (CPD)
* Demonstrable experience of autonomous clinical assessment and decision making
* Eligibility to work in the UK
* Willingness to undertake further training relevant to General Practice
* Evidence of training in minor illness, long‑term condition management, or urgent care
* Experience working within General Practice, Primary Care Network, or community healthcare
* Or demonstrable progress towards, or commitment to undertaking, the qualification
* Postgraduate qualification in Primary Care / Advanced Clinical Practice (MSc, PGDip, or equivalent)
Skills and Abilities
* Ability to support and manage a case‑load of patients autonomously, including assessment, planning, monitoring and evaluation, and providing health promotion and education to patients and carers.
* Ability to organise and deliver a high standard of evidence‑based clinical care, including developing and implementing policies and protocols.
* Ability to liaise effectively with members from other health care providers and primary and secondary care and external agencies.
* Ability to work under own initiative, prioritise own workload and work within a team.
* Ability to participate in clinical audit.
* Ability to recognise the limits of your own practice.
* Willingness to assist and advise on the development of the role.
