Job summary
We currently have a substantive Band 5 Community Mental Health Nurse full time (37.5 Hours) post available and are looking for a nurse to join our trust and bring fresh ideas and enthusiasm for developing the service.

Responsibilities
* Managing a caseload via face to face and telephone contacts
* Undertaking initial routine assessments on behalf of the service
* Attending outpatient clinic appointments
* Undertaking face to face visits in the community setting including home visits
* Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
* Attending regular MDT meetings and liaising with colleagues from other services (both internally and externally such as Social Care, Crisis Resolution and Home Treatment Team and third sector organisations)
* Medication administration
* Participating in the duty worker rota (Monday to Friday 09:00am to 17:00pm)
Qualifications
* Nursing Degree or equivalent
* Mentorship qualification or willingness to undertake training
* Audit exposure
* CPA implementation
* Experience in appropriate area
* Appropriate Sections of the Mental Health Act (1983)
* General principles of Health & Safety at Work Act
* Able to interpret NMC guidelines to personal liability
* Interest in research
* Ability to travel independently
