An exciting opportunity has arisen for creative and innovative individuals with excellent communication skills to join the city-wide Health Facilitation Team. Based in the north of the city, you will work to improve access to mainstream primary and acute services, promoting health-enhancing activities to improve outcomes for people with learning disabilities within the framework of level one and level two Health Facilitation.
Your responsibilities will include:
1. Contributing to the planning, delivery, and evaluation of care pathways for people with learning disabilities.
2. Facilitating training and education for the wider health economy.
3. Maintaining effective links with primary health care partners to support interventions that reduce health inequalities, including the implementation of health screening and health action plans.
4. Collaborating between primary and secondary services for adults with learning disabilities when accessing healthcare.
5. Working within a multidisciplinary team to support adults with learning disabilities and their carers, within the frameworks of health facilitation (levels one and two) and transition.
6. Coordinating clinical services under the supervision of the Specialist Nurse – Health Facilitation.
7. Ensuring effective communication among GPs, consultants, and other team members, and coordinating assessments, planning, interventions, and evaluations of care.
8. Liaising with professionals, carers, and service users regarding pre- and post-admission support to primary and acute services for individuals requiring palliative care.
