Overview
At Wirral Community Health and Care NHS Foundation Trust, we pride ourselves on providing the best possible care for our patients. We are looking for dynamic nurses to join our multi-disciplinary team in the Community Intermediate Care Centre (CICC). This is an exciting new initiative for the Trust, focusing on high quality, integrated care for patients from within the local adult population with sub-acute health care needs.
Responsibilities
* To work as part of a multi-disciplinary team, providing nursing and rehabilitation for in-patient beds.
* A member of the Community Intermediate Care Centre (CICC) who is skilled in assessing, planning and evaluating nursing care.
* In the absence of senior colleagues, take charge of the unit, manage patient admissions, provide care and discharge planning, whilst contributing to the effective working of the Unit.
* Work in partnership with multi-disciplinary team members, other professionals and agencies to meet the health and rehabilitation needs of patients within the Unit.
* Demonstrate professional skills and competence in care planning/coordination, medicines management, and care and treatment delivery.
* Demonstrate excellent interpersonal and communication skills to support patients with a wide range of conditions, helping them understand and, where possible, take on self-management of their condition.
* Be flexible and adaptable to working shifts covering 24 hours, 7 days per week as part of internal rotation.
* Supervise less experienced or qualified staff and students on placement; actively contribute to quality assurance processes and service development.
Requirements
* A registered nurse with current professional registration.
* Committed to delivering a high standard of care and supporting change management.
* An excellent communicator who will innovate, inspire and promote patient safety whilst supporting the ward team.
* An advocate for patients, able to identify changes in need and be instrumental in planning for their current and future care.
* English language proficiency, in line with Part 7 of the Immigration Act 2016 (proof of ability to fulfil all spoken aspects of the role in English).

Salary and conditions: The Trust pays staff in line with national NHS Terms and Conditions. Relevant experience may be considered when salary offers are made. For all part-time positions, salaries are pro-rata. Flexible working is supported. The Trust may offer redeployment priority where applicable.
