Main duties of the job
The post holder willsupport the practice to ensure the delivery of safe,effective and efficient systems for repeat prescribing, medicines optimisation,reducing medicines waste and maximising patient outcomes.
To recommend changes to prescribing or medicinesoptimisation systems and processes in practice. To resolve prescription-relatedissues and encourage better pharmaceutical understanding with patients andthroughout the primary health team, including GPs, nurses, practice staff, carehomes and community pharmacy. By working as part of the multidisciplinary team,your role will be to improve the quality and safety of all aspects relating toprescribing and medicines optimisation within the practice. You will alsocontribute to the training and development of practice staff

Job summary
The Pharmacy Technician's role is to support the practice to ensure the delivery of safe,effective and efficient systems for repeat prescribing, medicines optimisation,reducing medicines waste and maximising patient outcomes.

RESPONSIBILITIES
Providemedicines optimisation advice to Doctors, nurses and other healthcareprofessionals to promote the safe, effective, quality and cost-effective use ofmedicines.
Toreconcile medicines using the practice protocols for newly registered patientsand following hospital discharge or outpatient consultation. Work withpatients/carers and community pharmacists to ensure patients receive themedicines intended.
Carry outhousekeeping whilst in patient records e.g. Dose optimisation and medicinessynchronisation
Supportrepeat prescribing systems and processes in practice with a focus on qualityimprovement and supporting safe prescribing.
Coordinatemonthly prescriptions for care homes
Supportthe Acute Prescription clerks and care co-ordinator team by helping withqueries relating to prescription requests and look for ways to improve theefficiency and safety of the prescription issuing process, ensuring queries areresolved and scripts issued in line with the practice prescribing protocoltimeframe (usually 2 working days)
Deal withpatient medication queries via telephone call back e.g. dose schedule, sideeffects, adverse effects and out of stocks, swallowing difficulties, urgentrequests
Resolvequeries with prescriptions and issue certain medication where appropriate inline with practice protocols
Participatein practice clinical meetings, patient participation groups, and other meetingsto improve engagement of the role of pharmacy technician with the GP model andpromote issues relevant to prescribing and medicines optimisation
Helpimplement electronic prescribing and repeat dispensing in appropriate patientsincluding working with community pharmacy staff to identify appropriatepatients
Liaisewith medicine management & optimisation pharmacist on analysing patientprescribing data, including an awareness of prescribing budget and how this canbe managed
Assistpractice to reach QOF, QIPP and LCS enhanced service targets
To planwith Clinical team the implementation of projects and audits
Implementdrug withdrawals and alerts e.g. MHRA alerts
Managepractice formularies to improve the quality, safety and cost effectiveness ofprescribing and to reflect CCG prescribing policy
Assistpractice with audits, QI audits and reviews of the practice prescribingdashboard
Providemedicines related training to staff
Observethe principles of clinical governance when developing policies, SOPs,protocols, guidelines and procedures
