Overview
We are looking for an experienced SCPHN HV / SN, someone who has a specialist childrens nursing qualification or equivalent to join an expanding nursing team for Looked after Children within Leeds Community Health Trust. The post is full‑time and you will be working in our friendly and supportive nursing team caring for our children and young people who are looked after by Leeds city council, alongside their families and caregivers, to maximise their health potential and address their health inequalities.
Responsibilities
* Manage and monitor the delivery and healthcare of looked after children and young people aged 0–17.
* Work directly and indirectly with children, young people, families and carers to improve health outcomes.
* Provide support within a hybrid approach, working 50% of the time from base and delivering support across the West Yorkshire ICB area.
* Demonstrate an understanding of the challenges faced by the citys most vulnerable children and young people.
* Engage in multi‑agency and professional working to ensure excellent communication and self‑motivation in improving outcomes for looked after children and their families.
* Use excellent organisational and planning skills to manage your own caseload.
Qualifications & Experience
* At least 2 years post registration experience.
* Knowledge and experience in the field of looked after children and safeguarding.
* Specialist childrens nursing qualification or equivalent (SCPHN HV / SN).
