There are 2x roles available - 20 hours per week each. This could be 1 full time post for the right person.

The post holder will provide primarysupport to general practice staff with regards to prescription and medicationqueries. They will help support the repeat prescription system, deal withacute prescription requests, and medicines reconciliation on transfer of careand systems for safer prescribing, providing expertise in clinical medicinesadvice while addressing both public and social care needs of patient in theGP practice (s).

The post holder will provide clinicalleadership on medicines optimisation and quality improvement and manage someaspects of the quality and outcomes framework and enhanced services.

The post holder will ensure that thepractice integrates with community and hospital pharmacy to help utiliseskill mix, improve patient outcomes, ensure better access to healthcare andhelp manage workload. The role is pivotal to improving the quality of careand operational efficiencies so requires motivation and passion to deliverexcellent service within general practice.


Main duties of the job

Patientfacing Long-termcondition Clinics.See patients with singleor multiple medical problems where medicine optimisation is required (e.g.COPD, asthma). Review the on-going need for each medicine, a review ofmonitoring needs and an opportunity to support patients with their medicinestaking ensuring they get the best use of their medicines (i.e. medicinesoptimisation). Make appropriate recommendations to Senior Pharmacists or GPsfor medicine improvement.

Patientfacing Structured Medication Review. Undertake clinicalmedication reviews with patients face to face or virtually and producerecommendations for senior clinical pharmacist, nurses and/or GP onprescribing and monitoring.

Managementof common/minor/self-limiting ailments. Managing caseload ofpatients with common/minor/self-limiting ailments while working within ascope of practice and limits of competence. Signposting to community pharmacyand referring to GPs or other healthcare professionals where appropriate.

Patientfacing medicines support. Provide patient facing clinics forthose with questions, queries and concerns about their medicines in thepractice

Telephonemedicines support. Provide a telephone help line for patientswith questions, queries and concerns about their medicines.


About us

At Health and Beyond, we are able provide a wide range of healthcare clinics and services to our patients; our clinics and services are delivered by our GPs, Advanced Nurse Practitioners, Practice Nurses and other clinical staff including midwives.


Job responsibilities

We are looking to fill 2 roles:

1 is a 20 hour a week maternity cover role for 9 months and the other is a 20 hour a week permanent post. This can be 1x full time role if required for the right person

Main Duties &Responsibilities

Patient facing Long-term conditionClinics.See (where appropriate)patients with single or multiple medical problems where medicine optimisationis required (e.g. COPD, asthma). Review the on-going need for each medicine, areview of monitoring needs and an opportunity to support patients with theirmedicines taking ensuring they get the best use of their medicines (i.e.medicines optimisation). Make appropriate recommendations to SeniorPharmacists or GPs for medicine improvement.

Patient facing medicines support. Provide patient facingclinics for those with questions, queries and concerns about their medicinesin the practice

Telephone medicines support. Provide a telephone helpline for patients with questions, queries and concerns about their medicines.

Medicine information to practice staff and patients. Answers relevant medicine-relatedenquiries from GPs, other practice staff, other healthcare teams (e.g.community pharmacy) and patients with queries about medicines. Suggesting andrecommending solutions. Providing follow up for patients to monitor theeffect of any changes.

Unplanned hospital admissions. Review the use ofmedicines most commonly associated with unplanned hospital admissions andreadmissions through audit and individual patient reviews. Put in placechanges to reduce the prescribing of these medicines to highriskpatient groups.

Management of medicines at discharge from hospital. To reconcile medicines following dischargefrom hospitals, intermediate care and into care homes, including identifyingand rectifying unexplained changes and working with patients and communitypharmacists to ensure patients receive the medicines they need postdischarge. Set up and manage systems to ensure continuity of medicines supplyto highrisk groups of patients (e.g. those withmedicine compliance aids or those in care homes).

Signposting.Ensure that patients are referred to theappropriate healthcare professional for the appropriate level of care withinan appropriate period of time e.g. pathology results, common/minor ailments,acute conditions, long term condition reviews etc.

Repeat prescribing. Produce and implement a practicerepeat prescribing policy. Manage the repeat prescribing reauthorisationprocess by reviewing patient requests for repeat prescriptions and reviewingmedicines reaching review dates and flagging up those needing a review.Ensure patients have appropriate monitoring tests in place when required.

Risk stratification. Identification of cohorts ofpatients at high risk of harm from medicines through pre-prepared practicecomputer searches. This might include risks that are patient related,medicine related, or both.

Service development. Contribute pharmaceutical advice forthe development and implementation of new services that have medicinalcomponents (e.g. Enhanced access to care homes ,advice on treatment pathwaysand patient information leaflets).

Information management. Analyse, interpret and presentmedicines data to highlight issues and risks to support decision making.

Medicines quality improvement. Undertake clinical auditsof prescribing in areas directed by the GPs, feedback the results andimplement changes in conjunction with the practice team.

Medicines safety. Implement changes to medicines thatresult from MHRA alerts, product withdrawal and other local and nationalguidance.

Implementation of local and national guidelines andformulary recommendations. Monitor practice prescribing against the local healtheconomys RAG list and make recommendations to GPs for medicines that shouldbe prescribed by hospital doctors (red drugs) or subject to shared care(amber drugs). Assist practices in seeing and maintaining a practiceformulary that is hosted on the practices computer system. Auditingpractices compliance against NICE technology assessment guidance. Providenewsletters or bulletins on important prescribing messages.

Education and Training Provide education and training toprimary healthcare team on therapeutics and medicines optimisation.

Care Quality Commission Work with the general practiceteam to ensure the practice is compliant with CQC standards where medicinesare involved.

Public health To support public health campaigns. Toprovide specialist knowledge on all public health programmes available to thegeneral public.

CollaborativeWorking Relationships

Recognises the roles of other colleagues within theorganisation and their role to patient care

Demonstrates use of appropriate communication to gain theco-operation of relevant stakeholders (including patients, senior and peercolleagues, and other professionals, other NHS/private organisations e.g.CCGs)

Demonstrates ability to work as a member of a team

Is able to recognise personal limitations and refer tomore appropriate colleague(s) when necessary

Actively work toward developing and maintaining effectiveworking relationships both within and outside the practice and locality

Foster and maintain strong links with all services acrosslocality

Liaises with other stakeholders as needed for thecollective benefit of patients including but not limited to:

oGPs, nurses and other practice staff

oOther healthcare professionals including CCG pharmacists,pharmacy technicians, optometrists, dentists, health and social care teamsand dieticians etc.

oLocality / GP prescribing lead

oLocality managers

oCommunity nurses and other allied health professionals

oCommunity and hospital pharmacy teams

oHospital staff with responsibilities for prescribing andmedicines optimisation


Person Specification


Qualifications

* Knowledge, Skills and Experience Required
* Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
* Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio.
* Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice
* May hold or be working towards an independent prescribing qualification. Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate
* Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
* Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
* NB, it is anticipated level of qualification held may vary according to the level of position and the components of the role being carried out, see person specification for details.
