Overview

Inclusion & Support Worker

We are looking for individuals who can engage creatively with people with disabilities on a 1:1 basis, supporting them to identify opportunities and activities that reflect their interests, promote independence, and lead to meaningful outcomes. At PLUS, we are guided by the values of Respect, Compassion, Integrity and Dependability and we’re looking for people who live these values through their practice every day. Due to the requirements of the role, we particularly encourage applications from people based locally.

Could you be part of our team who put the young people and adults (up to age 35) first and work closely with families to plan and provide our support?


Responsibilities

* Build relationships with the individual/s and their families to support them and their son/daughter to participate in the planning, preparation and delivery of person centred/outcome focused leisure activities in a variety of settings or on occasion, in the individuals’ home.
* To brief and de-brief parents/carers before and after events and to complete all necessary forms or reports.
* To work collaboratively with staff at PLUS and from other organisations.
* Implement the individual’s support plan to achieve stated outcomes.
* Support young people with personal care such as toileting and feeding.
* Promote independence.
* Complete paperwork promptly (mostly electronic).


Qualifications and Skills

* Previous experience working with people with disabilities.
* Excellent communication skills and the ability to build a rapport.
* Good basic education.
* Please note: you will be required to become registered with SSSC and undertake relevant qualifications to enable you in your role.
* Highly Desirable: Driving license – with access to your own car.
