* Supported Living (Hampshire) Salary: £50,000–£60,000 per annum
* Job type: Full-time, Permanent
* Schedule: Monday–Friday, 9am–5pm (Hybrid: ~2 days WFH / 3 days community)
* Location: Hampshire
* Covering two supported‑living packages across the area (field‑based, regional travel)
As Clinical Lead RMN / RNLD, you’ll oversee two supported‑living packages in Hampshire for adults with learning disabilities, autism, and complex needs, one of which involves highly complex behaviours that challenge.
You’ll provide clinical leadership, drive governance and quality, and work closely with ICBs, commissioners, and multidisciplinary teams to achieve the best outcomes.
Key Responsibilities
Lead and stabilise complex LD packages, implementing escalation pathways and crisis/risk‑reduction plans.
Complete holistic and functional assessments, developing PBS‑led, person‑centred care plans with measurable outcomes.
Coach and mentor nurses and support staff, delivering supervisions, reflective practice, and competency sign‑offs.
Manage all aspects of clinical governance: incidents, investigations/RCA, safeguarding, documentation, and CQC readiness.
Liaise effectively with ICBs, CHC teams, and local partners to evidence outcomes and negotiate care package requirements.
Monitor clinical quality KPIs and implement continuous‑improvement actions to drive excellence in care delivery.
About You
Seeking a Clinical Lead RMN / RNLD with:
* NMC registration and 5 years post‑reg experience
* Proven leadership in supporting adults with MH/learning disabilities, autism, and behaviours that challenge
* Strong PBS, de‑escalation, and dynamic risk‑management expertise
* Confidence in working with commissioners and multi‑agency partners
* A full UK driving licence and flexibility to travel across Hampshire
