Join our dynamic Reablement Team based in Bath and surrounding areas. We're a short-term service supporting adults at home and in care settings, focusing on promoting independence through rehabilitation and equipment provision.
Key Service Areas:
1. Discharge to Assess: Supporting patients post-discharge after illness, injury, or surgery.
2. Admission Avoidance: Preventing hospital admissions within 48 hours.
3. Planned Therapy: Helping patients achieve SMART goals and self-management.
You'll work closely with acute services, GPs, nurses, and social care teams, alongside a supportive multidisciplinary team including OTs, PTs, therapy assistants, and coordinators.
Location: St Martins Hospital, Bath
Hours: Core hours 8am-4pm; service operates 8am-10pm, 7 days/week (rota includes weekends/bank holidays with enhanced pay)
Collaboration: Close ties with sister teams in Keynsham & Midsomer Norton

Main duties of the job
As a Reablement Occupational Therapist, you will:
1. Carry out therapeutic assessments, treatment, and rehabilitation with patients, using person-centred goals.
2. Provide an evidence-based occupational therapy service, of a high standard, to patients in a community setting, by undertaking comprehensive assessments, reassessments, care planning, rehabilitation, equipment provision, and therapeutic care and support.
3. Hold responsibility for your own caseload, working with Rehabilitation Therapy Workers to deliver therapeutic programmes.
4. Provide effective supervision to junior Occupational Therapists, Therapy Assistant Practitioners, Reablement Therapy Workers, Occupational Therapy Apprentices, and students.
5. Support management with developing the Reablement Service, in line with the NHS transformation plan.
