The Pharmacy Technician will work as part of the practice and wider Primary Care Network (PCN) multidisciplinary team to support medicines optimisation, improve prescribing processes and help patients get the best outcomes from their medicines.
The post holder will undertake patient-facing, technical and administrative medicines optimisation tasks, supporting safe, cost-effective prescribing and reducing medicines waste. The role includes supporting repeat prescribing processes, promoting electronic repeat dispensing, supporting medication reviews and medicines reconciliation, and contributing to quality improvement initiatives across the practice and PCN.
The Pharmacy Technician will work closely with the Clinical Pharmacist, GPs, community pharmacies and other healthcare professionals to ensure patients receive high quality, safe and effective care.
Main duties of the job
Undertake patient-facing and supporting roles to ensure effective medicines use through shared decision-making.
Support medicines optimisation including inhaler technique checks, medicines reconciliation and medication reviews.
Prepare patients for Structured Medication Reviews by organising monitoring tests.
Support medicines reconciliation for care home patients and during transfers of care.
Promote the use of EPS, eRD and online ordering.
Improve repeat prescribing processes and reduce medicines waste through effective ordering and returns.
Support antimicrobial stewardship to reduce inappropriate antibiotic prescribing.
Support reception teams in appropriately streaming prescription requests.
Provide training to staff on safe and legal handling of medicines.
Work with the Clinical Pharmacist to manage shared care and complex medicines queries.
Develop working relationships with community pharmacies and other professionals.
Support implementation of national guidance, NICE guidance and local prescribing policies.
Contribute to QIPP, QOF, IIF and locally commissioned services.
Support quality improvement, CQC requirements and medicines audits.
Provide patients with lifestyle advice, public health information and signposting where required.

In addition, the post holder willbe responsible for encouraging the development of better understanding of theprinciples of medicines optimisation throughout the practice teams andpromoting good practice in line with therapeutic developments. This will involveassisting the Practice & PCN in achieving national requirements, NICEimplementation and utilisation of medicines optimisation initiatives.
Key duties andresponsibilities
Clinical
Undertake patient facing and patient supporting rolesto ensure effective medicines use, through shared decision-making conversationswith patients.
Carry out medicines optimisation tasks includingeffective medicine administration (e.g. checking inhaler technique), supportingmedication reviews and medicines reconciliation. Where required, utiliseconsultation skills to work in partnership with patients to ensure they usetheir medicines effectively.
As determined by the Practice & PCN, supportmedication reviews and medicines reconciliation for new care home patients andsynchronising medicines for patient transfers between care settings, linkingwith local community pharmacies
Support the Clinical Pharmacist in Structured Medication Reviews (SMR) i.e. organise necessarymonitoring tests prior to SMR
Provide expertise to address both the public healthand social care needs of patients,including lifestyle advice, service information, and help in tackling local health inequalities.
Manage shared care protocols and liaise with ClinicalPharmacists for more complex patients.
Support initiatives for antimicrobial stewardship toreduce inappropriate antibiotic prescribing locally.
Technical and Administrative
Support the Practice & PCNmulti-disciplinary team to ensure efficient medicines optimisation processesare being followed.
Implement efficient ordering and return processes andreducing medication wastage.
Provide training and support on the legal, safe andsecure handling of medicines, including the implementation of the ElectronicPrescription Service (EPS).
Promotion of Electronic Repeat Dispensing (eRD) and online ordering
Develop relationships with other pharmacy professionalsand members of the multi-disciplinary team to support integration across healthand social care including primary care, community pharmacy, secondary care andmental health.
Support practice reception teams in streaming generalprescription requests, so as toallow GPs and clinical pharmacists to review the more clinically complex requests.
Support the implementation of national prescribingpolicies and guidance within GP practices, care homes and other primary care settings.
Support the Practice & PCN todeliver on QIPP agenda, QOF, IIF and locally commissioned enhanced services
Supporting quality improvement measures andcontributing to the Quality andOutcomes Framework and enhanced services;
Support the PCN in reviewing and developingpractice policies for CQCrequirements
Person Specification
Qualifications
* Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC)
* Meets the qualification and training requirements as specified by GPhC
* Evidence of continued professional development (CPD)
* BTEC/NVQ Level 3 in Pharmaceutical Sciences or equivalent
* Enrolled in or completed an approved training pathway such as PCPEP or MOCH
Experience
* Experience working as a qualified Pharmacy Technician in primary care, community or hospital pharmacy
* Experience supporting medicines optimisation and prescribing processes
* Experience working with patients, carers or in health/social care settings
* Experience using GP clinical systems
* Understanding of current healthcare policy within Primary Care Networks
