Job overview
An opportunity has arisen for a Patient Flow Coordinator on Doyle and Wilson. Doyle and Wilson is a medical ward specialising in medicine, staffed by a friendly, experienced team. We value patient‑focused care and excellent discharge planning as a core part of the service delivery.
As a Patient Flow Coordinator you will assess a patient’s usual and pre‑admission support needs, and liaise with the multidisciplinary team during admission to secure appropriate support for discharge.
Main duties of the job
Support the early and effective discharge of patients, including clinical patient contact, intake of support requirements, coordination with the nursing, medical and therapy teams, and maintaining communication with social services and private sector partners.
Assist the ward manager and multi‑disciplinary clinicians, demonstrating compassion, strong teamwork and communication, and flexibility to meet the evolving needs of the ward.

Person specification
Education & Qualifications
* Healthcare related experience
* Presently working within an acute NHS ward environment
* NVQ level 3 in Acute Care (acquired in a hospital setting) or equivalent experience
Skills
* Good communicator, able to work alongside others and follow instructions
* Able to organise own workload and use own initiative, IT skills
* Confidentiality and professional discretion
Experience
* Experience working in a care setting, with people or children or been a carer for someone
* NHS experience in an acute setting
