An excellent opportunity has arisen for a creative, dynamic Band 3 RSW, with a passion for providing patient-centred rehabilitation, to join the trust.

The role sits within the inpatient acute stroke team as part of the Tower Hamlets Stroke pathway. The role will be a full time 37.5 hours substantive role.

You will be part of a large, friendly, highly successful multidisciplinary team, based at the Royal London Hospital (RLH), aiding assessment and delivering rehabilitation to patients on our busy stroke ward. This role is available full time (37.5hrs/wk).

The team comprises Occupational Therapists, Physiotherapists, Speech & Language Therapists and Rehabilitation Support Workers who work alongside other health professionals which may include Dieticians, Clinical Psychologists, and the Medical and Nursing teams.

You will be responsible for supporting the therapy team to deliver highly specialist individualised treatment programmes to service users. You will carry out a wide range of physical tasks requiring a good level of physical fitness.

You will be responsible for assisting the Speech and Language Therapist, Occupational Therapists, Physiotherapists in providing daily care including the implementation of therapy programs and supporting patients with their daily care needs to help them progress towards their rehabilitation goals.

The ideal candidate will have excellent communication skills, plenty of initiative and enthusiasm, be confident using technology and can work as a part of the wider MDT.

Main duties of the job

To support the inpatient therapy teams (Physiotherapy, Occupational Therapy and Speech and Language Therapy) within the Acute Stroke Unit at the Royal London Hospital in providing high quality patient centered care:

·To assist therapists in the rehabilitation of patients by carrying out treatment plans developed by therapists and by progressing those treatment plans within agreed frameworks.

·To carry out patient assessment and treatment in a wide variety of settings including the ward and gym.

·To facilitate stroke survivors with self-management

·Work autonomously without direct supervision to support patients in their rehabilitation plans using advanced communication and therapeutic handling skills.

·Assist therapists in treatment requiring more than one therapist including those with complex therapeutic handling needs.

·Lead the delivery of Therapist prescribed group exercise and education programmes.

·To autonomously identify need for re-evaluation, progression and termination of interventions

·Modify specific patient treatment programmes by reassessing and progressing as appropriate where agreed by responsible Therapist

·To autonomously identify and order simple aids and adaptations as well as the ordering of Therapy prescribed equipment, aids and adaptations for stroke RSWs.

·To communicate effectively with therapists about patients’ condition.

·To record patient information in a SOAP note format.

·To support patient-centred rehabilitation in order to facilitate timely, safe discharge, reflecting individual choice and maximizing individuals’ independence

·To maintain the set level of stocks and the cleanliness and safety of equipment.

·To participate in site based data collection and reporting.

·To participate in audit under guidance of qualified staff, including participation in team quality improvement projects.

·Support the team to use effective equipment and stock management systems including storage of equipment and ordering of equipment in keeping with service requirements

·To liaise with external service providers to facilitate safe and timely discharge.

1. •Good standard of general education, to G.C.S.E. level or equivalent
2. •Evidence of recent relevant and appropriate training and CPD
