Overview
This exciting opportunity has arisen for Social Work practitioners to join Adults social care. We are looking for an organised and dependable team player who has a positive and flexible approach, with excellent communication and interpersonal skills, along with being enthusiastic and self‑motivated. You will be expected to prove your ability to work independently and to at all times make defensible decision‑making decisions, and exact your professional curiosity when offering support to Adult citizens; the collective delivery and embedding of whole system priorities using strength based whole system approaches will be essential, and you will be offered high level supervisory support at all times.
You must be able to apply the principles and process which accompany the 3‑Conversation Model and the Care Act 2014, to deliver on person centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on wellbeing, prevention, and early intervention to support.
Responsibilities and requirements
* Show partnership working with other professionals both internally and externally.
* Demonstrate your excellent application of the roles and responsibility of a social worker. Within your role, you will be expected to use innovation where appropriate to resolve challenges within your caseloads.
* Demonstrate your knowledge and experience of the Continuing Health Care process / appeals, and be skilled in Section 117 aftercare planning and reviews.
* Demonstrate your ability and experience in working through disputes on a professional basis with other disciplines and support other team colleagues in disputes where required.
* You must have a Social Work qualification and be registered with Social Work England.
