We are working with a client in the Wirral area who is looking to appoint a Care Navigator. This is a key frontline role, acting as the first point of contact for individuals seeking advice and support around adult social care. As a Care Navigator, you will provide clear information, guidance, and signposting to help individuals access the right adult social care, health, and community services. You will play an important role in ensuring people feel supported, informed, and listened to. Key Responsibilities Act as the first point of contact for adult social care enquiries Provide information, advice, and guidance to service users and carers Signpost and coordinate access to appropriate health, social care, and community services Liaise with professionals to support joined-up care Maintain accurate records in line with council policies and procedures Deliver a high standard of customer service Work flexibly across remote and office-based settings as required Key Requirements Experience in a customer-facing or advisory role (social care or health experience desirable) Strong communication and interpersonal skills Good organisational skills with the ability to manage multiple enquiries Ability to work independently and as part of a team A person-centred approach and commitment to high-quality support Contract Length: 3-6 months with a view to extend to an ongoing contract.
