Contact information Ann-Marie Ward, ann-marie.ward1@nhs.net Andrew Jones, andrew.jones2@nhs.net About the role Mid Yorkshire NHS Teaching Trust has been awarded new funding to implement and provide a dietetic-led Off-FP10 service for enteral (HEF) and oral nutritional support (ONS) across Wakefield Place. Working in partnership with Wakefield Health and Care Partnership and the ICS, this service will ensure all patients requiring nutritional support are referred to and reviewed/monitored by the dietetic team. This new development will provide a patient-centred, needs based service, which will allow seamless and more timely care for community patients who may require nutritional support intervention. The aim of the new service is to stop inappropriate prescribing of ONS (in line with NHS England intentions), ensure all patients receive timely and appropriate dietetic intervention and provided regular and ongoing training to all care homes across Wakefield Place. The service also aims to reduce health inequalities by ensuring improved access to dietetic support for all patients, regardless of location. The service has been commissioned on a permanent basis, so we are looking to recruit into all areas including qualified dietitians, dietetic assistants and a new admin team. These exciting new posts will support the band 7 in implementation of the service, while maintaining their own clinical caseloads. They will be part of a team that also includes band 5s, band 3s and admin. There are two band 5 posts available, one permanent, one 12-month contract. The post-holders will work across Wakefield Place, providing a specialist dietetic service for patients with nutritional support needs, both in their own homes and in Care Homes. This may be through face-to-face contacts or telehealth and may include presentations to Care Home staff. The initial work will be to scope ONS prescribing across Wakefield, stopping or amending inappropriate prescriptions, as required. The post-holders will be based in Wakefield Community but may be required to attend meetings at one of the 3 hospital sites in Wakefield (Pinderfields), Dewsbury and Pontefract. The post-holders may also work remotely for part of the week. The dietetic department is fully committed to CPD and clinical supervision. As we take A, B and C students from Leeds Beckett University, the post holders will also be expected to take an active role in supporting and supervising our student training program. To give talks and presentations, e.g. wound care, cardiac rehabilitation, care home training. Provide health promotion advice on study days etc. to health professionals and members of the general public. To be professionally and legally accountable and responsible for all aspects of own work, including the management of patients in your care. To develop and implement treatment plans for patients following assessment and analysis of diagnostic criteria, nutritional history, biochemical and clinical data, and social/psychological circumstances. To be an integral member of the team in setting up, maintaining and evaluating a fully operational Off FP-10 model in the Wakefield community working closely with colleagues of all bandings. To help support the patients in solving issues and admin related problems in relation to prescriptions of nutritional supplementation. To continually develop and maintain practical and theoretical skills through continuing professional development by attending relevant study days, tutorials, and through case load experience. To produce evidence-based information in various media for all users, To know the professional and personal scope of own practice and make appropriate and timely referrals to other agencies. To be able to use research, clinical reasoning, and problem-solving skills to identify the therapeutic benefit of client centred interventions and make professional judgements with individuals with diverse or complex needs, or multiple problems. To undertake visits and/or assessments in a variety of settings as required by the need of the service, including some wards, department, home or other community environments. To be involved in the triaging of referrals into the dietetic service prioritising and signposting as appropriate Give accurate feedback and advice to relevant staff i.e. GP, medical staff, nurses, coordinators, other health care professionals and family/carers. Work effectively as a member of the interdisciplinary team. To establish and maintain clear lines of communication with others to enhance good working relationships and seeking help where appropriate. To communicate complex information effectively with patients, carers and the wider MDT to ensure understanding of condition and dietary treatment. Encourage a culture of open, honest and transparent communication. To approve and countersign written documentation completed by non-qualified staff. To be responsible for the completion of appropriate departmental statistical records and data collection. To provide appropriate specialised written reports and/or recommendations regarding individual needs as required by the Intermediate Care team. To review and reflect on own practice and make effective use of regular supervision and appraisal, in line with the Trusts Appraisal and Clinical Supervision policies. To take an active role in the supervision of non-qualified staff including conducting annual appraisals. To be responsible on a day to day basis for the prioritisation and allocation of tasks to non- qualified staff as required. To take an active role in the facilitation of ongoing development within the Dietetic teams. To contribute to developments in service delivery within the Dietetic Service in line with National guidelines and government and/or Trust initiatives. To be involved with appropriate professional groups such as peer support networks, specialist sections etc. To be involved in the supervision and training of students who are on A, B and C placements with the Trust.
