Job Description
We are seeking an experienced Clinical Pharmacist who is an Independent Prescriber to support a service in Preston. This position requires an individual who is confident working autonomously and has current clearances related to drug misuse services and secure environments. The successful candidate will play a key role in delivering high quality medicines management, prescribing support and patient-centred care within a busy clinical setting.
Key Responsibilities:
* Provide clinical pharmacist support including medication reviews and medicines optimisation
* Prescribe safely and appropriately as an Independent Prescriber
* Manage patients with long term conditions and complex medication regimes
* Work closely with multidisciplinary teams to ensure effective clinical pathways
* Maintain accurate prescribing and clinical documentation
Essential Requirements:
* Fully qualified Clinical Pharmacist
* Independent Prescriber (IP) qualification
* Drug misuse clearance
* Prison clearance or eligibility to obtain it
* Strong clinical decision making and communication skills
