Summary
We are seeking a Community Nurse to join our team in providing exceptional care within the community. The position is based in various locations across England, allowing you to work flexibly according to your preferences.
Responsibilities
* Deliver high-quality nursing care to patients in the community
* Develop and implement individualized care plans
* Collaborate with multidisciplinary teams to ensure comprehensive patient support
* Monitor patient progress and adjust care plans as necessary
* Provide education and support to patients and their families
* Maintain accurate patient records and documentation
* Participate in training and development opportunities for continuous improvement

Qualifications
* Registered Nurse qualification with valid NMC registration
* Proven experience in community nursing or similar healthcare settings
* Strong leadership skills with the ability to inspire and motivate others
* Excellent communication skills for effective patient interaction
* Ability to manage time efficiently and prioritize tasks effectively
* Familiarity with developing care plans tailored to individual patient needs.
