Experienced practice nurses, new or recently qualifiednurses or nurses who have not worked in general practice before but areconsidering doing so.
We are able to enrol a new to General Practice Nurse intothe vocational training scheme which will allow for a year of supportedteaching and learning alongside working in the practice.

As a Practice Nurse you will be responsible for the deliveryof nursing services to the practice population. Supported by Senior Nurses thepost holder will deliver high quality nursing care within their scope ofcompetence, focusing on supporting patients to manage their long-term conditions,address health issues and promote health and well being by delivering educationand screening activities.
Main duties of the job
The working hours are 17 hours per week with rotas across both Helmsley Medical Centre and Terrington Surgery to be agreed.
MainDuties and responsibilities
Meeting patients nursing care needs as identified, this willusually be in the surgery but occasional home visits may also be required. Assess,plan, provide and evaluate nursing care to meet those care needs in thepractice population
Work collaboratively with the multi-disciplinary team tomeet the needs of patients and play an integral part in delivering the keyperformance indicators for Helmsley and Terrington Practices.
Provide general nursing care, including wound management,and chronic disease management, vaccinations and immunisations of babies,children and adults.
Provide women's health services including cervical screeningand contraception
Conduct health assessments and screenings.
Support patients with long-term conditions, providingeducation and self-management advice.
Participate in health promotion activities and initiatives.
Maintain accurate and up-to-date patient records.

Primary keyresponsibilities
The following are the core responsibilities of the Practice Nurse.There may be, on occasion, a requirement to carry out other tasks. This willbe dependent upon factors such as workload and staffing levels:
1. Be instrumental in the support of the Infection Prevention Control(IPC) lead and to work to the IPC requirements by providing audits, training,support and evidence of compliance to the practice manager
2. Support in the publication of the IPC annual statement inconjunction with the practice manager
3. Develop, implement and embed an effective care management programmefor the frail and housebound patients registered at the practice
4. Assess the needs of patients ensuring the provision of healthcareis appropriate, incorporating evidence-based practice
5. Develop, implement and embed health promotion and wellbeingprogrammes
6. Implement and evaluate individual treatment plans for chronicdisease patients that promote health and wellbeing
7. Identify, manage and support patients at risk of developinglong-term conditions, preventing adverse effects to patients health
8. As required, to provide routine nursing care to patients in accordancewith clinical based evidence, NICE and the NSF
9. As required, to diagnose and manage acute and chronic conditions
10. Provide wound care (ulcer/Doppler, etc.) to patients
11. Review patient medications to enhance compliance
12. Provide guidance to patients in the use of prescribed andover-the-counter medications regarding side effects and dosages
13. Co-ordinate the provision of travel medicine services
14. Request pathology services and process pathology results asrequired
15. Oversee the provision of chronic disease clinics, deliveringpatient care as necessary, referring patients to secondary/specialist care asrequired
16. Maintain accurate clinical records in conjunction with extantlegislation
17. Ensure the appropriate maintenance and use of chronic diseaseregisters
18. Participate in well woman and well man clinics
19. Ensure compliance with the practice chaperone policy
20. Assist GPs with minor surgery when required
21. Prioritise health issues and intervene appropriately
22. Support the team in dealing with clinical emergencies
23. Recognise, assess and refer patients presenting with mental healthneeds
24. Implement vaccination programmes for adults and children
25. Support patients in the use of their prescribed medicines orover-the-counter medicines (within own scope of practice)
26. Liaise with external services/agencies to ensure the patient issupported appropriately (vulnerable patients etc.)
27. Delegate clinical responsibilities appropriately (ensuring safepractice and the task is within the scope of practice of the individual)
28. Support the clinical team with all safeguarding matters, inaccordance with local and national policies
29. Understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately
30. Deliver opportunistic health promotion where appropriate
