JOB PURPOSE
To work alongside individuals who use Turning Tides’ services and/or homeless services by adopting a holistic and client led approach to help people recognise their strengths and achieve their ambitions. To promote self-advocacy and advocate on behalf of people (as appropriate) to ensure people’s dignity and rights are upheld and they are able to access and maintain the support they need.
To provide support within busy hub drop-in sessions and lead on developing groups (focusing on substance misuse and wellbeing).
To share good practice and advice staff on dual diagnosis and the barriers people face. To work in a Multi-agency approach both internal and external to enable effective joined up working between relevant support agencies. Specifically around substance misuse and impacts on wellbeing, as additional support to services.
To work collaboratively with partners and Mental Health Services from the statutory and voluntary sector, providing drop-ins and joint support/assessments. The Substance Misuse and Wellbeing Worker will be required to case co-ordinate a caseload of clients and facilitate key work & group work.
KEY RESPONSIBILITIES
Support Work
• Support for individuals who are homeless and insecurely housed and experiencing multiple exclusions/difficulties
• Develop and assist in the delivery of recovery and health interventions for people who are homeless/ insecurely/ temporarily housed and experiencing multiple exclusions/difficulties.
• Help people access appropriate support through promotion of self-advocacy
and advocating on behalf of people as appropriate.
• Deliver information and emergency subsistence to people who are street
homeless and insecurely housed locally.
• Co-ordinating a caseload of clients with other partner agencies.
• To have a good understanding of substances and their effects, particularly with
relevance to mental health and wellbeing.
• To have a good understanding of relevant legislation linked to Mental Health
and Homelessness.
• Utilise motivational interviewing, cognitive behavioural techniques and brief interventions in both one-to-one and group settings to promote choice, control, wellbeing and self-management.
• Work alongside people to complete strength based support plans and effective risk management plans that uphold people’s dignity and rights.
• Attend multi-agency meetings to find sustainable solutions based on what
people want.
• Involve clients in the design and delivery of services and promote opportunities for feedback on the quality of service provision.
• To manage referrals in line with funding stream processes.
RELATIONSHIPS
• Take a leading role in building relationships and developing reciprocal referral
arrangements with relevant partner organisations.
• Help develop and maintain relationships with statutory and voluntary sector Mental Health Services across West Sussex, including all relevant partners relative to the funding as an example CGL, Sussex Partnership Foundation Trust and Coastal West Sussex MIND.
• To promote and support joint assessments for people experiencing substance
use and Mental/Physical health difficulties as a result of this
MONITORING
• Maintain client database and monitoring systems including Turning Tides
Inform database.
• Complete quantitative and qualitative data when required for monitoring and evaluation purposes, including ensuring that all KPI’s are monitored and
reported on effectively.
GENERAL
• Abide by all Turning Tides’ policies and procedures and regulatory requirements at all times.
• Ensure all policies and procedures well embedded within your Service and that
your staff abide by them at all times.

• To consider co-production when working on all new initiatives or changes as and when they arise.
