Derbyshire Community Health Services currently has an exciting opportunity for a Support Worker Supervisor to join the Community First Team in Derby City.

Our dedicated teams work rotationally over seven days, ensuring that every individual receives the highest level of home care support. If you have prior experience in caregiving and a commitment to excellence, we want you on our team!

Based out of Florence Nightingale Community Hospital and Perth House Assessment Hub, you\'ll need the flexibility to travel to various sites where DCHS services are provided. Our shifts include weekends and bank holidays, offering you the opportunity to make a meaningful impact on a regular basis.

To be responsible for the day-to-day supervision of support workers.

The delegation of patient\’s visits to ensure an effective support worker service is provided to patients in a timely and cost-effective way.

To be a role model to the support worker team and actively monitor and maintain clinical skills and professional manner.

To communicate effectively with patients and carers to maximise rehabilitation potential, as appropriate and under supervision.

To work with qualified therapists and nurses in the assessment and treatment of patients with a variety of conditions and progress/modify intervention programmes within parameters set by the therapists and nurses.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people\'s own homes and via virtual consultations.
