Location: Community-based, across allocated local packages in Cornwall including St Austell, Bodmin, Perranporth, & Redruth.

Role Purpose
The Clinical Team Leader is a registered nurse who combines direct clinical delivery with day-to-day leadership of care teams within complex care packages.
This is a hands-on clinical role. The postholder will work directly with clients delivering clinical interventions while also supervising, supporting, and signing off competencies for Healthcare Assistants (HCAs). The role provides on-site clinical assurance, continuity, and leadership across one or more complex care packages within a defined local geography.

Core Responsibilities
1. Clinical Care Delivery
Deliver direct nursing care within complex packages in line with assessed clinical need
Undertake and review holistic nursing assessments and contribute to care planning
Deliver clinical interventions, monitoring, and escalation in line with best practice
Act as the clinical decision-maker on shift when present within the package

2. Leadership & Supervision
Act as Clinical Team Leader for allocated care teams
Provide visible leadership, acting as a role model for safe, person-centred care
Support, coach, and mentor HCAs within the package(s)
Undertake formal and informal supervision in partnership with the clinical team
Support induction and orientation of new staff into the package

3. Competency & Workforce Assurance
Deliver, assess, and sign off clinical competencies for HCAs
Ensure staff are competent before undertaking clinical tasks or lone working
Identify training needs and escalate to the Clinical Lead / Care Coordinator
Maintain accurate, auditable competency records

4. Clinical Governance & Quality
Ensure care is delivered in line with care plans, risk assessments, and policies
Maintain high-quality clinical documentation and daily records
Monitor practice, identify risks, and take corrective action early
Escalate safeguarding, incidents, or concerns in line with policy
Contribute to audits, reviews, and service improvements

5. Multi-Disciplinary & Stakeholder Working
Liaise with families, Case Managers, ICB and MDT professionals
Represent Libertatem professionally within clinical and review settings
Support the implementation of MDT treatment plans within daily care delivery
Provide structured updates to Clinical Leads, Care Coordinators, and Case Managers

6. Operational Support
Work collaboratively with Care Coordination to support rota effectiveness
Assist with stabilising packages during periods of risk or transition
Support cover planning within packages where clinically appropriate
Contribute to package reviews and escalation planning

Professional & Regulatory Responsibilities
Practice in line with the NMC Code of Conduct at all times
Maintain NMC registration and complete revalidation requirements
Keep professional knowledge current through CPD and mandatory training
Adhere to all regulatory, contractual, and organisational standards

Person Specification
Essential
Registered Nurse with active NMC registration (no restrictions)
Experience working with complex care clients (e.g. ABI, spinal injury, neurological conditions)
Confident delivering hands-on clinical care in community settings
Experience supervising, coaching, and assessing care staff
Strong understanding of clinical governance and risk management
Excellent communication and documentation skills
Full UK driving licence

Community or home-based complex care experience
Experience working across multiple packages
Exposure to both adult and paediatric complex care
Experience supporting MDT working and case management
