Overview
Your role will involve creating and maintaining meaningful relationships with young people with SEND and their families. As a PfA Life Coach you will engage with young people from the age of 14 to 25, providing support in four areas of Preparing for Adulthood: Independence, Health and Wellbeing, Community Inclusion, Employment. You will help young people to identify their skills & talents, try new things, and achieve their goals. You will maintain strong relationships with young people, their families, and local organisations. You will support young people and their families to navigate different services and processes to support great outcomes.
Responsibilities
* Engage with young people aged 14 to 25 and support them in Preparing for Adulthood across four areas: Independence, Health and Wellbeing, Community Inclusion, Employment.
* Help young people identify their skills and talents, try new experiences, and work towards goals.
* Build and maintain strong relationships with young people, families, and local organisations.
* Support families to navigate services and processes to achieve positive outcomes.
Requirements / Qualifications
* Hardworking, flexible, and committed to supporting disabled young people to achieve their goals and aspirations.
* Ability to build trusted relationships with young people, their families, and local organisations to pursue positive outcomes for all.
* Passionate about equity and inclusion and experience working with diverse groups of young people.
Details
Salary: Grade 6, £29,540 - £32,597 per annum.
Hours: 37 hours per week.
