Overview
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker, who is keen to deliver a range of quality care services to patients visited by the Community Health Support Team based with the Community Response Hub at Tarporley Hospital. The team can visit patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.

The hours are part time 19 hours per week. Consisting of 2 x 9.5 hour shifts, worked on a rota basis between 22.00 – 07.00 including Bank holidays and weekends.
The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.

Responsibilities
* Experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and providing care for palliative patients.
* This post involves Lone working as well as working as a member of a team.
* Travel across the Western Cheshire footprint is essential for work purposes, so transport for work MUST be demonstrated.

Induction and Training
All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role.
