The Role:
Joining our Accelerated Review Team as a Senior Case Worker is an excellent next step in your career in Social Work. You’ll be an integral part of our team providing individuals aged 65 and over with support and guidance to maximise their independence and wellbeing.
You will undertake high-quality reviews and assessments to determine eligible care and support needs. You will play a key role in ensuring care arrangements are appropriate, proportionate, cost-effective, and aligned with the principles of the Care Act 2014, least restrictive practice, and strengths-based working.
You will be flexible, resilient, and be able to work dynamically across Hampshire, responding to service pressures and supporting timely decision-making.
What you’ll do:
1. Complete Care Act assessments and reviews, analysing complex information to make person‑centred recommendations on care, eligibility, and service provision.
2. Review and adapt care packages at pace to promote independence, achieve outcomes, and ensure support is safe, proportionate, and least restrictive.
3. Work collaboratively with individuals, families, providers, health colleagues, and internal teams to deliver timely, high‑quality outcomes and support service objectives.
4. Maintain accurate case recording, contribute to team learning and quality improvement, and demonstrate flexibility to work across Hampshire and a range of care settings (home, residential, nursing placements).
What we’re looking for:
5. Significant experience in adult social care completing Care Act assessments and/or reviews.
6. Sound understanding of the Care Act 2014, eligibility, and strengths-based practice.
7. Confidence in working with complex cases, balancing risk, autonomy, and safeguarding.
8. Ability to work independently, flexibly, and at pace in a pressurised environment.
9. Strong analytical and decision-making skills with an outcomes-focused approach.
10. Excellent communication skills, including the ability to have sensitive and challenging conversations.
11. Willingness and ability to work across Hampshire, with travel between locations as required.
