Community Health Care Assistant
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant who is keen to deliver a range of quality care services to patients visited by the Rural Alliance Care Community team based at Chowley Oak Tattenhall.
Applicants MUST demonstrate how they will meet the travel requirement for this post.
The hours are 37.5 per week. The role involves visiting patients in their own homes to provide nursing care which includes venepuncture, wound dressings, care of diabetics and end-of-life care.

Main duties
As part of a friendly team, you should have experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience of providing care for palliative patients. This post involves lone working and requires travel across the Western Cheshire footprint; access to transport for work purposes is essential and MUST be demonstrated in your application.
Qualifications
* Appropriate related qualification - or willingness to complete an apprenticeship appropriate to the Clinical Support Worker role
* Completion of or working towards an apprenticeship and gaining qualification equivalent to level 3 or above in a subject relevant to the role of Healthcare Assistant
* To demonstrate a range of work procedures and practices and to exhibit your knowledge and abilities
* Completion of or commitment to complete the Care Certificate (CWP In-house 2- day training to be completed within 3 months of start date)
Knowledge and Expertise
* Understanding of need for confidentiality; empathy and sensitivity
* Basic wound management
* Basic risk assessments and record keeping
Experience
* The ability to demonstrate good quality care, whilst adhering to Trust policies and procedures, and working towards completing Care Certificate
* Experience of working collaboratively i.e., with colleagues, wider teams, service users etc
* Supporting others in a care setting, whether this is experience in a professional or lived experience (friend, family, voluntary etc.)
* Basic clinical skills including: venepuncture, skin care, wound care, clinical observations
* Experience of working in a community care setting including provision of personal care
