Overview

We are seeking mental health nurses to work in Carshalton & Wallington Integrated Recovery Hub (IRH) as Care Co-ordinator/Community Psychiatry Nurse. The role involves managing a caseload of adults with complex health and social care needs using the recovery model. You will facilitate care planning with individuals, including their mental, physical, and social care needs; undertake recovery goal setting; and provide regular Care Programme Approach (CPA) reviews. The post involves working closely with other teams and services within the borough, the Trust, partner agencies, and non-statutory organisations; writing social circumstances reports for mental health review tribunals and attending reviews.

You will be committed to working creatively to provide a community service for people experiencing mental health problems. You will be well organised, able to work autonomously, have excellent written and oral communication skills, and be a team player. The team works closely with Sutton and Cheam Integrated Recovery Hub, and the post also requires you to represent duty across both teams on a regular basis. A full driving licence and access to your own vehicle are required.


Responsibilities

* Work autonomously within the parameters of the role, leading and supporting junior staff and the wider care team to deliver quality nursing care, addressing physical, psychological, social, recreational and spiritual needs within a recovery-focused framework.
* Lead on activities to ensure the fundamentals of care are met (nutrition, hydration, hygiene, comfort, emotional and social support).
* Coordinate and evaluate collaborative, patient-centred care; seek feedback and improve plans with the multidisciplinary team (MDT).
* Liaise with MDT and partner organisations in a positive, collaborative manner.
* Administer and monitor medication side-effects; monitor service user compliance; ensure safe custody of medicines, sharps, and clinical equipment; report incidents according to trust policy.
* Lead and support/teach junior staff on risk assessment and risk management, using evidence-based practice and clinical judgement.
* Support and supervise junior staff to develop care management: initial data collection, monitoring progress, providing feedback and discussion; produce reports and in-care reviews; devise care plans with service users, carers and MDT.
* Coordinate junior staff and the wider care team in providing hands-on care and evaluating care processes; undertake 1:1 engagement with service users in an honest, open, non-judgemental manner.
* Recognise and value service users as individuals, maintaining respect and dignity; ensure care respects equality and diversity.
* Monitor and record clinical observations (e.g., physical observations and vital signs); escalate or treat as appropriate; promote healthy lifestyles and provide health promotion advice.


Requirements

* Registered Mental Health Nurse with post-registration experience; community mental health experience preferred, but applicants with other backgrounds may be considered if they demonstrate aptitude and ability.
* Range of clinical skills in assessment, crisis intervention and care coordination.
* NHS AfC Band 6. A commitment to flexible working is welcomed.
