Team Manager – Integrated Hospital Discharge Service

Hackney Council is currently recruiting for an experienced Adults Team Manager to join the Integrated Hospital Discharge Service.
This is a key leadership role within a fast-paced, multi-disciplinary environment, focused on ensuring safe, timely, and effective hospital discharges for Hackney residents.
About the Role
As Team Manager, you will provide strong operational leadership across the service, managing a team responsible for complex discharge planning. You will play a critical role in reducing Delayed Transfers of Care (DTOC) while ensuring residents and their families receive high-quality, person-centred support throughout their discharge journey.
You will work collaboratively with health and social care partners, driving effective multidisciplinary working to achieve positive outcomes.
Key Responsibilities
* Lead, manage, and support a team of social workers and practitioners
* Oversee complex discharge planning and decision-making
* Drive performance to reduce delayed transfers of care (DTOC)
* Ensure timely completion of statutory assessments and interventions
* Promote high-quality, outcome-focused practice across the service
* Work closely with multidisciplinary teams to coordinate effective discharge pathways
* Support residents and families throughout the hospital discharge process
About You
We are looking for a confident and experienced leader who:
* Has extensive experience in hospital discharge and complex case management
* Demonstrates strong operational leadership and team management skills
* Has a sound understanding of statutory frameworks, including:
* Mental Capacity Act
* Care Act
* Adult Safeguarding
* Possesses excellent communication skills and the ability to engage effectively with multidisciplinary teams
* Thrives in a fast-paced environment and can prioritise competing demands
* Is committed to delivering high-quality, person-centred outcomes
