Complex Care Nurse

We have an opportunity for a Band 7 Complex Care Nurse to join our Complex Care Nursing service in Tunbridge Wells.
Our complex care nursing service provides proactive and responsive care to support patients with frailty and other complex long-term conditions, such as Heart Failure, COPD, and diabetes.
We support patients with robust management plans to promote self-management, where possible, and to optimise long-term condition management and prevent unplanned hospital admissions.
Main duties of the job
Each nurse in our team is responsible for managing and delivering nursing care to a defined patient caseload within an aligned geographical area.
Your patients could be of any age, but they are often elderly, recently discharged from hospital, terminally ill, or have physical disabilities.
As a Band 7 Complex Care Nurse, you will work as part of a team to keep people safe and well in Maidstone. You will use your clinical judgment to support evidence-based care planning and ensure that service users have a positive experience.
Additional Working Pattern: 7.5 hrs 3 days/week, with 1 weekend in 4.

Person Specification
Qualifications
* Registered healthcare professional (NMC)
Post Graduate Qualifications
* Evidence of postgraduate specialist qualifications relevant to practice (e.g., non-medical prescribing, mentorship, coaching, clinical expertise)
Experience
* Experience managing health-related projects or complex caseloads
Skills & Abilities
* Effective time management, prioritization, and working under pressure
* Ability to communicate complex and sensitive information effectively to individuals and groups
