This involves providing appropriate, professional Social Work support for adults with mental health issues and carers, focusing on enablement, recovery, and achieving the social care outcomes outlined in the Department of Health's Adult Social Care Outcomes Framework and the social work in mental health paper from the College of Social Work. By adopting an enablement approach and working within an integrated service, we aim to offer personalised, outcomes-led support based on structured, person-centred assessments that inform the development of suitable enablement packages of support, and adhere to the principles of the mental health enablement model. Additionally, we will undertake complex assessments, packages of care, and long-term support for individuals who qualify for specialist input. Role-specific responsibilities To provide personalised support services, appropriate assessment tools and techniques are utilised to establish cost-effective and relevant options for individuals. This process includes facilitating self-assessments and creating support packages that aim to maximise long-term independence while minimising reliance on health and social care services. Risk assessments are conducted for adults and carers, and appropriate referrals are made in accordance with eligibility criteria and Mental Capacity legislation. Additionally, Mental Capacity Act/best interest assessments are conducted in compliance with statutory guidance to ensure that adults have the opportunity to demonstrate capacity. The allocation of caseloads is managed in compliance with statutory requirements and professional best practices, taking into account individual choices. Group work is facilitated using the recovery and enablement model of mental health, and effective working relationships are established with local services, specialist teams, Mental Health Trust, NHS Barnet, Hospital Trusts, and the 3rd sector to deliver holistic and seamless services to clients. Attendance at service user forums and contribution to engagement forums are also included in the role. When an individual has a dual diagnosis, the assessment and support plan are coordinated with a range of professionals to take into account the differing needs. Understanding and experience in operating with a comprehensive assessment framework, including self-assessment. Experience in working with people with complex mental health issues. Experience in preparing clear and accurate correspondence, reports, and other written documentation. Understanding and experience in working with people with a mental health issue within a care environment. Experience in working with people within a social care model of mental health and collaborating with health professionals to meet the medical and physical needs of individuals. Demonstrated high performance in successfully delivering a customer-focused service to challenging targets and objectives. Experience in actively and effectively participating in cross-functional networks and local groups.
