About The Role

We are looking for a dedicated Community Support Worker to join our team. The successful applicant will support local Hounslow residents to access various community‑based activities using person‑centred approaches with clear pathways for residents to progress towards independence. This Role is a community‑based role. 80% of your week will be spent in the community supporting people in different settings.


About The Team You’ll Be Working In

The Community Access Service (CAS) supports people with learning disabilities and autistic spectrum conditions to lead full and purposeful lives.

The service focuses on enabling our service users (residents) to become active citizens in their community, empowering them to achieve their goals and develop their social skills.

We deliver a range of projects, activities/outings and community‑based drop‑ins during the day, evenings and weekends, as well as virtual sessions and online support using Zoom.


Some Of The Co‑produced Opportunities We Provide Include:

* Our Drop‑in and advocacy services
* Animal Care allotment and Farm group
* Arts & Crafts, Cooking, Drama, Dance
* Health and Wellbeing Awareness
* Cultural Awareness of Celebrations & Festivals
* Sports/Athletics, Self‑Defence, Swimming and Yoga
* Social outings – free jumping, amusement parks, cultural visits, companion cycling
* Online Zoom Sessions – cookery, exercise, speaking up groups

The Community Access Service consists of a team of 4 Community Support Workers who report directly to the Community Access Team Leader.


About You

The right person for the role should have:

* Experience of working in supporting people with learning disabilities to explore new opportunities, access local resources and identifying areas which will enable individuals to flourish.
* A good understanding and commitment to safeguarding vulnerable adults/adults at risk.
* An understanding of barriers people with disabilities face in relation to accessing community activities, including public transport, with clear creative and innovative strategies for overcoming these.
* The ability to work flexibly and be self‑motivated, organised, physically fit and can lone work in the community, whilst managing own workload and communicating effectively.
* Relevant Health and Social Care qualification (desirable)
* Maths/ English Grade C or above at GCSE (essential)

* Competent in applying the principles of personalisation in the delivery and development of the Community Access Service projects, drop‑ins and sessions, maximising participation of residents in attendance.
* Can support and enable individuals to meet their social needs by providing and developing a wide range of opportunities including volunteer work, access to community resources, leisure and recreation activities, outreach, travel training, sessional activities.
* Can Support Residents to access and be fully integrated into the community as positive role models that celebrates diversity and inclusion.
