30 hours per week.

Are you an experienced social care professional passionate about developing others and promoting high quality practice? We are seeking an Advanced Practitioner to join the Livewell Referral and Support Service (LRSS) and play a key leadership role in delivering excellent, person centred social care.

LRSS is the single access point for priority health and social care services, ensuring people tell their story only once. Our multidisciplinary team triages and manages referrals from the public and professionals, resolving enquiries wherever possible and directing people swiftly to the right support.

The ideal candidate will be a highly competent social care professional, aligned with national practice standards, and skilled in reflective, evidence informed approaches. You will also play a key role in promoting personalisation, ensuring practice reflects statutory requirements, legal frameworks, strengths based principles, and the core values of adult social care.


Main duties of the job

Provide professional supervision, mentoring and practice leadership to Social Workers and Community Care Workers.

Offer specialist advice on complex casework, safeguarding, risk management and personalisation.

Ensure high-quality assessment, support planning and documentation through quality assurance and oversight.

Lead on work allocation, workflow management and team performance, contributing to service targets and improvement.

Support development of services through policy implementation, audits, research and service improvement initiatives.

Build strong relationships with service users, carers, and partner professionals, often handling sensitive or complex situations.

This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

To provide practice supervision to a range of qualified and non qualified staff working within adult social care

To promote continuous professional development and the delivery of high quality support to users by ensuring staff undertake and access appropriate development opportunities

To provide advice and assistance to social workers and community care workers in relation to complex case work

To support staff to manage their caseloads as well as managing the emotional demands of their workload

To facilitate and support the professional development of social care staff by providing professional supervision

To provide a clear and responsive practice leadership role within the team including demonstrating best practice mentoring and promoting the highest professional standards via peer reviews and audits

To manage performance in line with organisational and service requirements in collaboration with senior and practice management including the implementation monitoring and review of practice improvement action plans for individuals and groups of staff

Sustained concentration required when providing supervisions and practice guidance ensuring staff work within legislation national and local procedures This will entail managing competing priorities demands prioritisation and negotiation

Conflicting information may require the post holder to use judgement to assess a particular situation and decide on the best resolution and advise the worker accordingly

To identify any concerns in relation to risk performance practice wellbeing attendance and mandatory training Ensuring organisational policies and procedures are followed

Responsibility for financial and physical resources

Implementation of adult social care duties under the care act and delivery of the scheme of delegation

Oversight of workforce and recruitment at a team level to be supported by operational and senior management

Expectation for the post holder to understand the financial position of the organisation and service to recognise any impact or potential impact on service delivery

Undertake delegated financial tasks to support operational responsibilities within the team for example expenses claims staff equipment requests community equipment service requests and monitoring the physical resources assigned to that team which may include mobile phones personal alarms laptops and remote working equipment

Responsibility for administration

To allocate work across the team daily ensuring consideration of risk effective flow of work and achievement of departmental targets for productivity and quality

Audit and maintenance of documentation in line with professional standards

Oversight and appropriate response in line with organisational customer services policy to include complaints concerns and compliments

Ensure system and process is in place to receive customer and patient feedback and action and share learning including i want great care reviews

Responsibility for ensuring system and process in place for monitoring and signing off e roster and shifts monitoring sickness leave and training requests as well as documenting skills in line with practice supervision and line management policies

To quality assure assessments and support plans

To participate in case reviews and assist with deprivation of liberty investigations

Delegated responsibility on a day to day basis for supporting staff with issues such as protection of vulnerable adults procedures promoting personalisation and risk enablement

To undertake duties based on current professional knowledge as well as legislative and statutory requirements

High level of professional competence in health and social care practice recognising national standards adopting a person centred and reflective approach

To maintain service knowledge and expertise to contribute to the development of services including best practice systems and protocols which support the delivery of a quality service

To support staff in relaying information to service users and carers where proposed support does not meet expectations requiring effective communication skills judgement empathy and negotiation skills

Travel to visit service users homes attend meetings and court

To have the skills to chair and lead a variety of complex meetings with appropriate outcomes

Responsibility for implementation of policy and service developments

Assist operational and strategic management to develop the service to meet key performance requirements
