To provide autonomous nursing care in the community, ensuring a high standard of individualised evidence based and family centred care.
· To facilitate a seamless transition from hospital to home for babies and their and families in accordance with Newborn Care Unit/ Neonatal Outreach guidelines, whilst adhering to the OUH Trust Lone working policy.
· To provide education and support to families in the community.
· To work as a member of the multi-disciplinary team, communicating effectively with all levels of nursing, medical and other health care professionals, both hospital and community based.
· To partake in the supervision and preceptorship/mentorship of pre-registered students on spoke placements.
· To adhere to NMC Code
