We are seeking a motivated, enthusiastic andcompassionate Health Care Assistant to join our team. The successful candidatewill join our forward thinking and inclusive practice. You will join theNursing team and work closely with the GPs, ANPs, Practice Nurses and othermembers of the multidisciplinary team in providing a range of patient careactivities, ensuring the highest standards of care and service delivery.

Your role will be to provide vital support tothe medical team, assisting with a range of duties that include clinicalprocedures, patient care, and administrative tasks.You will work underthe guidance of nurses and other healthcare professionals, focusing on tasksthat free up doctors' time and improve the overall patient experience.

Job responsibilities
KeyResponsibilities But Not Limited To:
Clinical Duties:
Conduct blood pressure monitoring and new patient healthchecks.
Conduct the pre assessments for longterm conditions
Carry out electrocardiograms (ECG)
Assist in minor surgical procedures and contraceptivecoil fittings.
Administer vaccinations andimmunizations as per practice protocols.
Carry out home visits to ourvulnerable patient cohort
Support the clinical team in themanagement of the care homes we oversee
HealthPromotion:
Advise patients on health care, minor ailments, healthpromotion, and illness prevention.
Support chronic disease management clinics, includingdiabetes and cardiovascular health.
AdministrativeDuties:
Prepare and maintain clinical rooms andequipment before, during, and after patient care activities, ensuring a cleanand safe environment for patients.
Ensure the Emergency Medication is adequately stocked andfit for purpose.
Ensure proper handling and dispatch of laboratoryspecimens, including blood and urine samples.
Maintain accurate patient records anddocumentation.
Ensure fridge temperatures are regularly monitored andmaintained.
Participate in practice meetings and contribute to practicedevelopment
Support the training and mentoring of junior staffmembers.
Person Specification
Qualifications
* NVQ level 2
* Proficiency in phlebotomy.
* Excellent communication and interpersonal skills, and be confident to deal with patients, families and carers.
* Ability to work independently and as part of a multidisciplinary team, using initiative within the level of competence for the position.
* Commitment to patient confidentiality and data protection.
* Accuracy and a strong desire for attention to detail
* NVQ Level 3 in Health and Social Care or equivalent qualification.
* Experience in chronic disease management.
* Training in administering immunizations and vaccinations.
* Familiarity with EMIS & Docman clinical system.
Experience
* Experience working as a Health Care Assistant in a healthcare setting,
* Experience of working in General Practice.
