Main Responsibilities
Act as an Enhanced Care Practitioner (ECP) within a multidisciplinary Hospital at Home team. Key duties include:
* Providing clinical leadership and hands‑on support for assessment, management, and treatment of patients in community settings, with a focus on Urgent Community Response (UCR) referrals.
* Coordinating specialist, person‑centred care across primary, secondary, acute, and social care, ensuring seamless care from referral through to discharge.
* Managing the Hospital at Home clinical caseload and supporting colleagues via supervision, development and collaborative working.
* Contributing to service development through audit, learning, and reflective practice, supporting continuous improvement and a positive learning environment.
* Building and maintaining strong relationships with Care Coordination, Therapies, Nursing and partner teams throughout Bath and North East Somerset, Swindon and Wiltshire.
Qualifications
* Current registration with the NMC or HCPC.
* Recognised teaching or mentorship qualification.
* Significant experience in elderly care, frailty, community services or relevant acute care specialties.
* Clinical and technical skills in managing a range of complex conditions.
* Proven experience in urgent/acute care assessment and advanced clinical assessment qualification.
* Experience working as part of a multidisciplinary team.
* Excellent written and verbal communication skills.
* Self‑motivated, flexible, able to work autonomously with minimal supervision.
* Strong judgement, analytical skills and the ability to interpret complex situations.
* Adaptability to a constantly changing environment.
* Good IT skills and knowledge of clinical systems.
* Experience leading and supporting a multidisciplinary community team.
* Training in clinical supervision and/or leadership skills.
* Experience implementing change.
* Degree level education or equivalent experience/knowledge.
* Independent prescribing qualification (or working towards) with proven prescribing experience in frail older adults.
* Clean driving licence and access to a vehicle for daily use.
* Involvement in relevant clinical interest groups.
* Experience with clinical audit, research and quality improvement.
