Overview
We are seeking an experienced, high‑performing Social Worker for our Wandsworth Hospital Team, located at the Wolfson Unit, Queen Mary’s Hospital, Roehampton, with occasional work at St George’s Hospital. The role supports patients with complex physical, cognitive, and communication impairments, and works within a multidisciplinary team to facilitate safe and timely discharge.
About the Role
* Provide detailed assessments, manage risks, and make clear, evidence‑based decisions.
* Manage referrals, support patient flow, and act as a safeguarding enquiry officer.
* Facilitate complex discharges, working with carers and local authorities.
* Attend weekly multidisciplinary meetings and fortnightly Goal Planning sessions.
* Support patients from different boroughs, ensuring effective communication across local authorities.
* Represent the service effectively to external partners and provide liaison with housing applications and local authorities.
About You
* Post‑qualification experience and strong knowledge of relevant legislation and statutory duties.
* Expertise in supporting patients with physical, cognitive, and communication impairments.
* Excellent communication, decision‑making, and time‑management skills.
* A collaborative approach and commitment to positive outcomes for residents.
* Current registration with Social Work England.
* Proactive, solution‑focused attitude and strong interest in health integration.
