Specialist Clinical Pharmacists – Emergency Care, AMT and Frailty (LW)
Join NHS Scotland as a Specialist Clinical Pharmacist in Emergency Care and Frailty to deliver high‑quality care and service innovation across acute and community settings.
About the Role
There are 1 permanent and 1 fixed‑term (12‑month) posts funded to expand the acute clinical pharmacy service and support organisational priorities around frailty, hospital at home, outpatient antimicrobial therapy and complex discharge. The posts will be based at Victoria Hospital and support both inpatient and outpatient services.
Key Responsibilities
* Support early clinical review, discharge and medicines optimisation of frailty patients within the emergency department and admissions wards.
* Develop and expand the acute clinical pharmacy service for A+E, hospital at home, outpatient antimicrobial therapy and complex discharge.
* Provide direct support to the new acute frailty unit, delivering clinical reviews, medicines optimisation and discharge planning.
* Work collaboratively with multidisciplinary teams, including pharmacists, technicians, foundation pharmacists and senior clinical pharmacists.
* Contribute to service innovation and continuous improvement initiatives.
* Maintain clinical expertise through ongoing training, postgraduate framework and independent prescriber qualification.
Qualifications & Experience
* Valid pharmacist registration with NHS Scotland.
* Experience in acute or community pharmacy is preferred but not essential; training and support will be provided.
* Demonstrated commitment to patient care, service development and a growth mindset.
* Ability to work independently and as part of a multidisciplinary team.
* Previous experience in frailty, antimicrobial therapy or discharge services is an advantage.
Additional Requirements
* Eligibility to work in the UK and, for overseas applicants, possession of a valid visa or potential for sponsorship.
* Membership of the Protection of Vulnerable Groups (PVG) scheme prior to appointment.
* Will be subject to NHS Scotland pre‑employment checks, including identity and work status verification.
