We are looking to recruit a suitably qualified and registered Advanced Practitioner under ARRS to lead our multidisciplinary team on behalf of Harthill Primary Network member practices.

This is a new role - you will have experience as an Advanced Practitioner in one of two key specialities - nursing or paramedicine. You will be a masters qualified registered level 7 practitioner with an independent prescribing qualification looking to move into or already working in a community focused role.
Main duties of the job
On a day-to-day basis you will take both a clinical leadership role - solving complex and more challenging diagnosis and provide supervision to support growth - enhance services and reduce risk. In this role you will build capacity by successfully influencing challenging and leading. Your approach alongside your knowledge skills and experience will make a tangible and quantifiable difference across the PCN. You will be valued and feel a strong sense of job satisfaction as you work to deliver Primary Care Services with our NHS partners.
About us
We are one of seven PCNs across the East Riding and cover a registered population of around 18,000 patients across a semi-rural area. While we have areas on our patch which are among the 10% least deprived, we have pockets of deprivation and challenges to the prevention and management of long-term conditions for our population. Frailty is a significant issue; our population is older than the England average. Harthill PCN practices provide services for 330 care home beds with the support of our existing ARRS and practice colleagues.
Yorkshire Health Partners Ltd (YHP) is managing the recruitment, employment and leadership of people under NHS Englands Additional Roles Reimbursement Scheme (ARRS) for Harthill PCN.
YHP is a GP led organisation established in 2012. It currently has 19 member practices across the East Riding and one in Hull with a combined list size in excess of 318,000 patients. It was initially established to support independent partner led general practice by holding contracts for primary care as well as focusing on the principle of care closer to home. YHP seek to provide a strong voice for primary care, patients, and staff, both as a separate entity, and in partnership with others.

As an advanced practitioner this post holder will lead our team of Additional Roles Reimbursement Scheme - ARRS - First Contact Practitioner - FCP - colleagues working collaboratively across the PCN to play a crucial role in clinical governance building capacity and capability and providing resolution and critical thinking to some challenging patient diagnosis or issues. This role will primarily work initially with our care home residents but will support development plans for a wider Proactive Care Team across our practices.

The PCN will greatly benefit from your level of knowledge skills experience and leadership to build capacity and capability across the network.
Key Areas of Responsibility
To lead the multi-disciplinary team across the PCN to assess and triage patients, including same day triage.
Provide definitive treatment such as the prescription of medications in accordance with guidelines - locally and nationally - and make referrals to other members of the primary care team.
Manage undifferentiated undiagnosed conditions and identify red flags and underlying serious pathology and take appropriate action.
Undertake complex decision making to inform the diagnoses investigation and complete the management of episodes of care within a broad scope of practice.
Proactively take a personalised care approach and population centred care approach to enable shared decision making with the presenting patient.
Leading on MDT work to achieve on the PCN DES associated IIF KPIs and related QOF indicators.
Building capacity through effective leadership
Provide clinical supervision to others in clinical roles within primary care such as first contact practitioners and personalised care roles. Risk Management and Quality Assurance
Ensure robust clinical governance is in place for your practice which includes regular audit and evaluation supervision and CPD.
Management of workload and personal development
Act on own initiative in prioritising and managing workload delegating tasks appropriately and referring to designated MDT and Board Members in respect of clinical managerial and administrative areas as necessary.
Maintain own personal and professional development as agreed at your annual review.
Develop and deliver services which puts the patients of Harthill PCN at the heart of what we do.
Support the next stages of the PCN Development into an INT.
