The Care Home Nursing Team within Glasgow City HSCP (part of NHS Greater Glasgow and Clyde) is recruiting to support 93 care homes across Glasgow City. The post involves partnership working with other health professionals, commissioning and social work to improve care aligned with health and social care standards and care assurance outcomes.
The shift pattern for this post is Monday- Friday 9am-5pm, however service review may require changes in hours.
The role offers the opportunity to support care homes with practice development and training, and to deliver clinical expertise to drive improvements in nursing care intervention areas including infection prevention and control, tissue viability, palliative care, person-centred care, nutrition and hydration, and falls prevention. A key part of the role is carrying out care assurance visits to all 93 care homes in Glasgow City HSCP.
There will be an emphasis on professional development and support to care homes to deliver clinically effective services in line with governance and organisational development strategies of the service.
Professional requirements include being a 1st level registered nurse with a minimum of 2 years post-registration experience and with evidence of ongoing professional development. Experience and knowledge of the care home sector are advantageous. The post holder should demonstrate expert knowledge across nursing practice and the ability to implement improvements that may challenge existing practices.

Responsibilities
* Provide/deliver integrated, collaborative and coordinated services to support nursing, residential and adult residents in care homes, and to support care home staff and managers.
* Work closely with Commissioning and Social Work Services.
* Carry out care assurance visits to the 93 care homes in Glasgow City HSCP.
* Support practice development nurses in the delivery of training and education to care home staff and managers.
Qualifications
