<div><h3>Diabetes Practice Nurse – Greenway Community Practice</h3><p><b>Position: Diabetes Practice Nurse (Part-Time, 16-20 hours per week)</b><br/>Greenway Community Practice | NHS Pension | 27 days holiday pro rata</p><p>We are seeking a motivated and compassionate Diabetes Practice Nurse to join our friendly and supportive general practice team. This role will involve running structured diabetes reviews alongside delivering routine treatment room nursing duties.</p><p>This is an excellent opportunity for a nurse with experience or a strong interest in diabetes care who is keen to develop their skills within general practice.</p><p>The successful candidate will:</p><ul><li>Undertake structured diabetes reviews for patients with Type 1, Type 2 and pre‑diabetes in line with NICE guidelines</li><li>Provide patient‑centred education and lifestyle advice to support self‑management</li><li>Support diabetes medication reviews and escalation of care where appropriate</li><li>Deliver treatment room duties including wound care, injections, immunisations, venepuncture and other routine procedures</li><li>Work collaboratively with GPs, nurses and the wider multidisciplinary team</li><li>Maintain accurate clinical records and contribute to QOF and audit requirements</li></ul><p>We are looking for a nurse who:</p><ul><li>Is a Registered Nurse with current NMC registration</li><li>Has experience in primary care and chronic disease management</li><li>Is confident undertaking treatment room duties</li><li>Has diabetes training or experience</li><li>Has excellent communication skills and a patient‑focused approach</li><li>Works well independently and as part of a team</li></ul><p>This is an excellent opportunity for a local Practice Nurse who is committed to providing high‑quality care in a primary care setting.
