About The Role
Were looking for a dedicated Substance Misuse Practitioner to join our integrated healthcare team, supporting prisoners aged 18+ with drug, alcohol, and low-level mental health needs.
Youll deliver tailored, trauma-informed interventions, from 1:1 sessions to group programmes, helping individuals prepare for release and reintegrate into the community with confidence and support.
What Youll Do:
Deliver structured 1:1 and group sessions for substance misuse and mental health
Create and manage care plans, assessments, and harm reduction strategies
Work closely with housing, employment, healthcare, probation, and more
Support smooth transitions from custody to community
Keep accurate records to track outcomes and meet KPIs
What Youll Bring:
Experience supporting vulnerable individuals in a structured setting
A collaborative, person-centred approach
Strong organisational and communication skills
Fluency in Welsh (essential)
