What the Job Involves As a Social Worker in the All Age Disability Service, you will work in the Young Adults Team, focusing on assisting young people aged 16-25 with disabilities. Your role involves assessing risks, identifying needs, and coordinating services to enhance the welfare of vulnerable individuals. Collaborating with multi-agency networks, you will develop and implement plans to help young people transition from Children’s Services to adulthood, ensuring their educational, employment, and independent living needs are met. Additionally, you will maintain accurate records, participate in ongoing professional development, and work outside core hours when necessary. Skills Professional qualification in social work with current Social Work England registration Experience in statutory social care and safeguarding Specialist knowledge of social work theories and practices Excellent communication and interpersonal skills Strong organizational abilities and multitasking skills Ability to work collaboratively in a multidisciplinary team Proficient in IT, including Microsoft Office Assessment skills for complex family situations Liaison and negotiation skills with agencies and service users Ability to travel throughout the county
