The post is for a Band 6 Acute Liaison Learning Disability Nurse who will be based in Morriston Hospital and work into Neath Port Talbot Hospital and Singleton Hospital to facilitate reasonable adjustments to enable people with Learning Disability to access health care.

Main duties of the job

The role will involve training staff within the hospital in the use of the Hospital Care Bundle, communicating with and assessing people who have a Learning Disability, what making reasonable adjustments entails, supporting the person and their carer’s and liaising with the wider MDT in the community.

The role will require collaboration with colleagues in neighbouring Health Boards to ensure audit and data collection is comparable and links to Improving Lives and ensuring better outcomes for people accessing hospital.

Develop links between the Liaison Service and all clinical areas within Acute and Secondary health provision in Health Board.

Work in collaboration with Acute/Secondary Care; Primary Care; CLDT’s; AATU’s and other key stakeholders from health, social care and other agencies.

Contribute to the development of care pathways to increase awareness of and reduce impact of health inequalities experienced by people with Learning Disabilities.

Contribute to the identification of barriers to access to healthcare and develop action plans to address.

Frequently receive and communicate highly complex and sensitive information to/about people with Learning Disabilities, their families, carers and other professionals, to ensure the needs of individuals are met.

Provide guidance, support and education to people with Learning Disabilities, their families and carers accessing healthcare when required.

Qualifications and Experience

Essential criteria

1. Registered Learning Disability Nurse
2. NMC registrant
3. Qualified to degree level
4. Evidence of commitment to ongoing professional and personal development
5. Understanding of the mutli-agency input into Learning Disabilities Services, and experience of working in a Community Team.
6. Knowledge and understanding of the additional health needs of People with Learning Disabilites; Mental Capacity Act; reasonable adjustments to ensure access to healthcare.
7. Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh
8. Knowledge of the role and function of Acute/Secondary Care settings.

9. Ability to build and promote effective working relationships with a diverse range of professionals, agencies, and relatives.
10. Ability to analyse and communicate complex data and information to support decision making processes.
11. Established communication, negotiation and influencing skills; verbal and written. Proven ability to motivate and influence others.
12. Proven ability to implement plans of care in relation to people with Learning Disabilities who have additional health needs
13. Able to work hours flexibly dependent upon service need Able to travel between sites across the Delivery Unit
14. Satisfactory Standard/Enhanced DBS clearance including an Adults and Childrens Barred List check
15. Excellent interpersonal skills in working with all stakeholders.
16. Ability to demonstrate excellent skills in leadership and collaboration.
17. Innovative, creative and forward thinking.
