An exciting opportunity has arisen for an experienced RMN to join the Specialist Community Forensic Team, based within the Adult Forensic Service as a forensic community psychiatric nurse.
The Specialist Community Forensic Team (SCFT) is a community based service in Greater Manchester providing support to individuals with mental health issues relating to their offending/risk behaviour who have been discharged from a secure inpatient setting.
The role is based across the Greater Manchester area as a Band 6 Forensic Community Psychiatric Nurse within SCFT.
Responsibilities
* Hold a small caseload of patients who are already residing in the community or are about to be discharged from a secure setting.
* Conduct assessments, robust risk management, and care planning with specialist interventions to support patients who may be subject to MOJ conditions or probation interventions.
* Provide specialist nursing interventions and advice as required.
* Monitor, evaluate and update care plans to ensure patient progress and the effectiveness of interventions; participate in CPA planning reviews.
* Be part of a robust and supportive multidisciplinary team (MDT) where risk is reviewed and feedback is provided regularly.
* Offer supervision to junior staff members of the team.
* Support patients in transitioning from inpatient secure hospital into the community.
Requirements / Qualifications
* Experienced RMN with willingness to work in a fast-paced, demanding environment; flexibility in practice is essential.
* Ability to work as part of a team and to manage a small caseload with patients who have a history of offending behaviours and/or are subject to MoJ or probation/license conditions.
* Senior clinical experience in forensic or community mental health settings.

Hours, location and contacts
The team operates Monday to Friday, 9:00–17:00. This role involves working with patients across the Greater Manchester area.
