Primary Care Network Health and Wellbeing Coach

The aim of the Health and Well-beingCoach will be to develop collaborative relationships and to empower people totake control of their own health and wellbeing.

The post holder will be expected to playa dynamic role in supporting patients to adopt healthier lifestyles and tocoach and mentor individuals to achieve personalised health careobjectives. In undertaking this role thesuccessful candidate will be required to demonstrate effect team working and astrong commitment to delivering the Abbey Health PCN agenda to deliverexcellent care for all patients within the PCN.

The successful candidate will be empathetic,reflective, self-aware, and enjoy working with a wide range of people. Theywill have good communication and negotiation skills and able to provide a highquality, non-judgemental service. They will be proactive with a flexibleattitude, a good team player and committed to developing themselves in thisskilled role. Health and Wellbeing Coaches will have access to ongoingsupervision, skills development, and support so they are able to continue todevelop and gain experience.

The successful candidate will work under the supervision of anAHP Clinical lead, providing patient care at our PCN sites.


Main duties of the job

The Health and Wellbeing Coach provides personalised, one-to-one and group coaching to support people with long-term conditions to take control of their health and wellbeing. Working closely with GPs and the wider multidisciplinary team (MDT), the coach helps patients identify what matters to them, set meaningful goals, and build confidence in self-managing their care. This includes promoting behaviour change, supporting self-management education and peer support, and linking patients with community-based services. The role also involves using IT systems and referrals to identify suitable patients, managing a caseload, and contributing to MDT meetings. The coach supports colleagues across the Primary Care Network (PCN) by sharing expertise in personalised care approaches and helping embed coaching across services. Key to the role is reducing health inequalities and supporting patients to live more independently, while promoting positive risk-taking, shared decision-making, and improved health outcomes.

It comprises of 2 practices, The Maltings Surgery in StAlbans, and Summerfield Health Centre in London Colney. The Maltings Surgery is located in the centre of the city of St Albans witha growing population of approximately 19,000 patients. Summerfield Health Centre is located in London Colney, one of the biggestvillages in Britain with a population of nearly 10,000 residents. London Colneyhas a lively and cosmopolitan community. Summerfield Health Centre has recentlymoved into a new, fit for purpose, health centre that creates a hub ofcommunity services. Job responsibilities

Primary Duties and Areas of Responsibility

The main dutiesand responsibilities will be to:

Provide one-to-one and group health coaching support for peoplewith one or more long-term conditions, based on what is important to them, withthe aim of: improving peoples levels of activation, empowering people tomanage their own health and improve their health outcomes.

Develop collaborative relationships and work in partnership withhealth, social care, and community and voluntary sector providers andmulti-disciplinary teams to holistically support patients wider health andwell-being, thereby contributing to the reduction of health inequalities.

Provide education and specialist expertise to PCN staff,supporting them to improve their skills and understanding of personalised care,behavioural approaches and ensuring consistency in the follow up of peoplesgoals with MDT input.

Work with the GPs and other primary care professionals within thePCN to identify and manage a caseload of patients.

Be the first point of contact at GP practices where coachingservices will be delivered, and to receive and manage referrals.

Where required and as appropriate, refer people back to otherhealth professionals within the PCN, in accordance with the needs, prioritiesand any urgent support required by individuals.

Using existing IT and MDT channels to screen patients, with an aimto identify those that would benefit most from health coaching

Provide personalised support to individuals, their families, andcarers to support them to be active participants in their own healthcare.

Facilitate and coach patients, both individually and in groups, tomanage their own health and wellbeing and live independently through:

coaching and motivating patients through multiple sessions toidentify their needs, setting goals, and supporting patients to achieve theirpersonalised health and care plan objectives;

providing interventions such as self-management education and peersupport;

supporting patients to establish and attain goals that areimportant to the patient;

supporting personal choice and positive risk taking while ensuringthat patients understand the accountability of their own actions and decisions,thus encouraging the proactive prevention of further illnesses;

working in partnership with the social prescribing service toconnect patients to community-based activities which support them to takeincreased control of their health and wellbeing; and

increasing patient motivation to self-manage and adopt healthybehaviours

Work with patients, both individually and in groups, to help themunderstand their level of knowledge, skills and confidence when engaging withtheir health and well-being and subsequently supporting them in shareddecision-making conversations.

Utilise health coaching skills to support people with lower levelsof activation to develop the knowledge, skills, and confidence to manage theirhealth and wellbeing, whilst increasing their ability to access and utilisecommunity support offers.

Work as part of a Multi-Disciplinary Team, participating inprogress meetings, ensuring that the MDT is kept fully up to date with progressreports.

Actively engage and support the design and promotion of healthpromotion campaigns.

Underpinning expectations:

The post holder is accountable to the PCN Clinical Director. Theyare professionally responsible for their own actions and for the actions ofothers under their supervision.

The Post Holderis expected to:

Adhere to PCN policiesand procedures and those of the constituent practices where relevant

Adhere to dataprotection regulations, data sharing and respecting confidentiality ofpatients, colleagues and the Primary Care Network

Undertake aDisclosures and Barring Service (DBS) application

Be committed tosafeguarding and promoting the welfare of children, young people and vulnerableadults

Maintain personaland professional development in order to maintain their skills

Promote equalityand diversity in a non-discriminatory way

To travelindependently between work sites and to attend meetings hosted by otheragencies.

Confidentiality:

The post holder must maintain theconfidentiality and security of information about patients, staff and the businessof the PCN and both partnerships in accordance with the Data Protection Act2018 and Caldicott principles.

Health & safety:

The post-holder will support andimplement on the full range of promotion and management of their own andothers health, safety and security as defined in the practice Health &Safety policy, the practice Health & Safety manual, and the practiceInfection Control policy and published procedures.

This will include (but will not belimited to):

Using personalsecurity systems within the workplace according to Practice / PCN guidelines.

Identifying therisks involved in work activities and undertaking such activities in a way thatmanages those risks across the business.

Making effectiveuse of training to update knowledge and skills and initiate and manage thetraining of others.

Using appropriateinfection control procedures, maintaining work areas in a tidy and safe way andfree from hazards, and initiation of remedial / corrective action where needed.

Keeping own workareas generally clean, identifying issues and hazards / risks in relation toother work areas within the business, and assuming responsibility in themaintenance of general standards of cleanliness across the business inconsultation (where appropriate) with other site facility managers

Undertakingperiodic infection control training (minimum annually).

Demonstrate dueregard for safeguarding and promoting the welfare of children.

Qualifications

* Holds or currently studying for a recognised healthcare or coaching qualification (or equivalent relevant experience in motivational interviewing)
* Good standard of education with excellent literacy and
