An exciting opportunity has arisen to provide strong leadership to a dynamic and friendly Older Adult Bury and HMR Community Liaison Team based in the center of Bury at the Ribchester Centre

We are looking for a motivated individual who has the ability to lead the day to day operation of the Older Adult Community Liaison Team, providing management and supervision to the integrated health and social care team.

If you are passionate about making a genuine difference to the lives of Older Adults by providing a comprehensive and specialist mental health service to people with both functional and organic illnesses within a community setting and have a focus on providing safe, effective high quality care then we would be delighted to receive your application.

Applicants should be a Registered Mental Health Practitioner (RMN) with current registration with substantial experience as a Band 6.

The post holder will have responsibility for the management and supervision of the Bury and HMR Older Adult Community Mental Health Team based at Ribchester Centre Bury.

You will provide strong leadership to the team to ensure it performs in the most efficient and effective way ensuring all key performance indicators are met whilst providing a safe and high quality service.

Detailed job description and main responsibilities

• To oversee the management of the Bury and HMR Older Adults Community Liaison caseload, leading the nursing team and co-ordinating the clinical and therapeutic care within
community, on a day to day basis:
• To take full responsibility for managing the quality of care provided to clients in the community and to ensure that appropriate supervised care is provided at all times.
• To chair clinical meetings as required
• To set and monitor standards of care with the team to review care and treatment of patients and to ensure appropriate standards are maintained.
• To participate in the development of care pathways to ensure the best possible service to clients.
• To ensure evidence based tools and interventions are used as appropriate.
• To support colleagues with complex clients management and joint work where required.

• To provide clinical leadership and robust decision making to ensure that
appropriate care pathways are identified for service users.
• To ensure all appropriate client records are maintained to the required Trust standard.
• To liaise with all agencies, and members of the multi-disciplinary team to ensure the needs of the client are met.
• To provide management and supervision of specialist practitioners and
practitioners.
• To manage productivity within the service.
• To lead the development of service standards and clinical audit
• To lead the development and implementation of new initiatives, incorporating researched based practices / evidence based practice.

Education / Qualifications


Essential criteria

* Professional qualification in Health / Social Care, as appropriate
* Experience of working with Older Adults in Health/ Social care setting
* Post registered or post graduate relevant study / equivalent mental health community practitioner experience
* Willingness to undertake further training relevant to the specialist practitioner role resulting from legislative or service changes, as appropriate
* Preparation for mentorship or equivalent


Desirable criteria

* A1-NVQ Assessors award.
* ECDL

* Practitioner experience across a range of mental health services
* People management experience
* Experience of robust clinical decision making and manage crisis situations
* Experience of liaising with partner organisations and agencies
* Providing Supervision
* Providing specialised advice in relation to mental health care
* Managing complex cases within the CPA framework


Knowledge


Essential criteria

* Extensive and Specialist knowledge of Mental Health and Social Care
* Care Programme approach
* Mental Health legislation


Skills and Abilities


Essential criteria

* Ability to manage complex clients and care packages as an experienced Care co-ordinator
* Ability to supervise and support practitioners in managing complex clients
* Ability to provide a high level of professional expertise in the care of clients with complex needs.
* Ability to provide specialist advice to practitioners.
* Ability to provide appropriate education and training to mental health practitioners as appropriate
* Excellent written and verbal communication skills
* Ability to communicate informally and develop supportive relationships with clients, carers and other professionals
* Ability to communicate formally in meetings and in other contexts
* Ability to prepare, record and complete relevant documents and reports to a professional standard.
* Ability to work effectively as part of a multi-disciplinary team
* Ability to undertake a full range of physical interventions e.g. moving & handling to support people’s personal care needs & MVA
* Ability to demonstrate effective time management and disciplined selforganisation
* To implement and utilise the supervision process.
* Ability to value diversity and respond constructively to discriminatory behaviour

* Ability to travel within the requirements of the post
* Ability to work flexibly to respond to service need
* Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs
