Overview
NELFT North East London Foundation Trust is recruiting for a Band 6 District Nurse/Senior Community Nurse to join our integrated community team in Thurrock. The closing date is 03 November 2025.
The service operates from 07:00 to 19:00 hrs, including regular weekends on a rota basis.
Please note this is not a community mental health role.
Main duties of the job
The post holder will support peers and the team leader and be an effective team member while working on their own initiative. They will supervise pre-registration students. They will have a commitment to collaborative working, actively supporting and liaising with other health and social professionals and agencies. The post holder will lead and manage a Community Nursing Team, geographically aligned to GP practices, and be responsible for the management of a caseload to ensure the provision of high standard nursing care to patients and their families in the community.
The post holder will work as part of the integrated care team, operating within and across professional and organisational boundaries. They will liaise with GPs, other professionals and statutory/voluntary agencies. The caseload manager works without direct supervision and provides leadership to the community nursing team.

Starting with NELFT

Induction is up to two weeks starting on the first Monday of each month and includes mandatory training, systems training and equipment allocation. Induction may include meetings with the executive team and senior managers, and attendance at drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - Fringe
This post attracts a High Cost Area Supplement of 5% of the basic salary (minimum payment £1,303 and maximum £2,198 per annum, pro rata for part time).

Person Specification
Experience
* Has experience of teamwork that supports effective work planning and delivery
* Minimum 1 year post-registration community nursing experience
* 3 years post-registration experience
* Experience of ensuring practice is based on sound evidence and clinical audit
Teaching
* Evidence of teaching and assessing learners
* Experience of interagency collaborative working
Skills
* Excellent range of clinical skills
* Good organisation skills and ability to prioritise
* Basic IT awareness and skills
Leadership
